Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

40-06-4    Basic - Employee personal items stored in or above a food preparation area. Cell phone on prep counter by radio.
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Hole in back door.
36-24-5    Basic - Hole in or other damage to wall. Hole in wall behind stand mixer.
12B-13-4    Basic - Opened employee beverage container in a cold holding unit with food to be served to customers.
25-05-4    Basic - Single-service articles improperly stored. Pizza to-go boxes on floor in back store room. **Corrected On-Site**
03F-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Sauce.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 19 between wall and grease trap located beneath 3 compartment sink, 4 on wheel of reach-in cooler, 1 between wall and shelf support beam over reach-in cooler, 9 in broken section of cove molding beneath handwash sink, 4 in drain by handwash sink.
41-02-4    High Priority - Toxic substance/chemical stored by or with food. Hand antiseptic and eye drops on shelf over deli-style reach-in cooler.
