A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Employee wearing Bracelet while preparing food. **Warning**
Basic - Observed approximately 43 dead roaches on premises: 10 dead roaches in Refrigerator at entrance to sushi bar, 15 dead roaches in and around gasket of freezer top of refrigerator at entrance to sushi bar, 5 dead roaches in black mini refrigerator, 10 dead roaches along wall with Three Compartment Sink, 3 dead roaches near paint cans at rear corner of kitchen. **Admin Complaint**
Basic - Single-service items for customer self-service not properly protected to prevent contamination. Unwrapped, sanitized, chopsticks provided in open containers on dining room tables. Advised that wrapped utensils may be left in bulk on tables. **Warning**
High Priority - Pesticide/insecticide labeled for household use only present in establishment. Two cans of Raid ant and roach spray labelled for residential use onsite. **Warning**
High Priority - Roach activity present as evidenced by live roaches found. Observed approximately 27 live roaches on premises: 2 live roaches in dining room, 2 live roaches in sugar bin at entrance of kitchen, 1 live roach in black mini refrigerator, 1 live roaches beneath toaster oven, 15 live roaches in gasket of refrigerator at entrance to sushi bar, 1 live roaches on floor of sushi bar, 1 live roaches on wall of sushi bar, 2 live roaches in microwave, 2 live roaches at Three Compartment Sink. **Admin Complaint**
High Priority - Roach excrement and/or droppings present around gaskets of all Reach In Coolers, around electrical outlet near Three Compartment Sink, along clean equipment racks. **Admin Complaint**
Intermediate - Establishment advertised white tuna on the menu/menu board but served another type of fish. White tuna offered on to go menu. Owner states escolar is the white tuna. **Admin Complaint**
