Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-10-4    Basic - Floor soiled behind pizza oven , stove and fryer along the wall. **Warning**
36-31-4    Basic - Holes in ceiling over pizza oven where pipes run thru. **Warning**
36-24-5    Basic - Holes in wall behind water heater at floor level. Small holes in wall from old shelving above water heater. **Warning**
33-33-4    Basic - Large amount of unused equipment/supplies present. Multiple coolers inside and outside the building. **Warning**
23-05-4    Basic - Soil residue build-up on top of lower pizza oven (between top and bottom units). **Warning**
33-34-4    Basic - Storage area not maintained clean and organized. (Area under front counter. Receipts are piled on lower shelf where rodent chew marks/nesting is observed. Open boxes of loose items surrounded by droppings. **Warning**

03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. (48°F cut tomatoes, 48°F cheese, 50°F sliced ham all in upper make table area. Upper lid was not properly closed. Lid was fully closed after taking temperatures. 35A-09-4    High Priority - Presence of insects observed crawling on the front line hand sink thru the silicon seal at the wall. Approximately 16 very small ants. Garbage can area and front bar counter also have heavy ant populations. Too many to count. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. (Approximately 8 semi-soft rodent droppings along the wall behind the pizza oven. 25 soft to semi soft on top of and around the water heater near the 3 bay sink. Approximately 8 observed dried along the wall under the mop sink and 3 compartment sink. operator is cleaning up droppings during inspection. 6 semi-soft on equipment shelves and floor area next to floor mixer. 12 dried under black shelf rack in back kitchen. 4 observed under glass door cooler viewed from back of unit, and unable to reach for squeeze test. 1 dried dropping on plastic of opened package of unfolded pizza boxes in hallway. 6 dried droppings on shelf in hallway next to folded, ready-for-use pizza boxes. 7 dried droppings on hallway floor near back door. In excess of 50 soft to semi-soft droppings under front counter on 2 shelves.) **Warning**
35A-21-4    High Priority - Rodent burrow or rodent nesting materials present. Observed large volume of chewed up ketchup packets on top of water heater and behind water heater. Water heater is below a shelf in a corner and is hard to observe area without effort. Operator removed shelf above unit to clean up packets. Pile of paper receipts under front counter with some chewed on. **Warning**
22-02-4    Intermediate - Accumulation of dried food debris/grease on multiple knives hanging on wall magnet next to sandwich maketable cooler. **Warning**
22-22-4    Intermediate - Encrusted material on can opener blade. **Repeat Violation** **Warning**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Neat 3 compartment sink. **Corrected On-Site** **Warning**
02C-04-4    Intermediate - Operator is not properly tracking/marking the number of days ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. (Cooked spaghetti, meatballs sausage.) **Repeat Violation** **Warning**
