Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Plates near entrance to kitchen not inverted. **Repeat Violation**
35A-03-4    Basic - Dead roaches on premises. 3 dead roaches in cabinet near 3 compartment sink. 7 dead roaches in cabinet under steam table, 4 egg casings on dish machine. 1 dead roach inside 2 door reach in cooler, 2 dead roaches inside gasket in two door reach in cooler. 3 dead roaches in cabinet under microwave. **Repeat Violation**
08B-49-4    Basic - Employee personal food not properly identified and segregated from food to be served to the public. Employee beverage and food items in top shelf of reach in cooler on cook line. **Repeat Violation**
36-22-4    Basic - Floor area(s) covered with standing water. Standing water on floor of walk in cooler. Frozen water on floor of walk in freezer.
36-17-4    Basic - Floor tiles missing in mop sink area outside kitchen.
14-40-4    Basic - Nonfood-contact equipment in poor repair. Cabinets in kitchen in poor repair. Holes and other damage to cabinets. **Repeat Violation**
33-16-4    Basic - Open dumpster lid.
06-05-4    Basic - Potentially hazardous (time/temperature control for safety) food thawed in standing water. Chicken thawing in three compartment sink in water. Operator moved to reach in cooler. **Corrected On-Site**
14-67-4    Basic - Reach-in cooler gasket torn/in disrepair. Two door reach in cooler on cook line. **Repeat Violation**
23-05-4    Basic - Soil residue build-up on nonfood-contact surface. Outside of employee food box in walk in cooler.
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Drain pipe . leaking into bucket under 3 compartment sink. Over the dish machine pipe leaking.
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. 1 #10 can of black eyed peas severely dented in dry storage room.
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. In two door reach in cooler raw bacon55°F , raw shelled eggs ambient 54°F, heavy whipping cream 48° F and sliced American cheese 48°F. Operator moved all items to walk in cooler. **Corrective Action Taken**
08A-05-4    High Priority - Raw animal food stored over ready-to-eat food. Raw shell eggs over milk and tea.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 1 live roach in cabinet near 3 compartment sink. 4 live roaches in can opener on prep table. 1 live roach in upper cabinet molding over toaster and microwave. 10 live roaches under prep table on cook line. 3 Live roaches under cart with clean dishes at end of cook line . 2 live on bait station under table in area outside walk in cooler. 1 live roach on plastic push cart under handle on end of cook line.
35A-23-4    High Priority - Roach excrement and/or droppings present. In cabinet above 3 compartment sink in corner also in utensil box in same cabinet.
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Hose Bibb at mop sink exterior of kitchen. **Repeat Violation**
02C-03-4    Intermediate - Commercially processed ready-to-eat, potentially hazardous (time/temperature control for safety) food opened and held more than 24 hours not properly date marked after opening. Heavy whipping cream not date marked.
22-22-4    Intermediate - Encrusted material on can opener blade. Rust corrosion and 4 live roaches in commercial can opener.
53A-01-5    Intermediate - Manager lacking proof of food manager certification. A list of accredited food manager certification examination providers can be found at http://www.myfloridalicense.com/dbpr/hr/food-lodging/managercertification.html. Operator on warning/ time extension from previous inspection due on 4/7/16. **Repeat Violation**
16-34-4    Intermediate - No quaternary ammonium chemical test kit provided when using quaternary ammonium chemical sanitizer at three-compartment sink/warewashing machine.
