Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

Several dead roaches observed in three outlet boxes mounted to the bottom corner of the cookline main make table. 1 dead in the dish machine. 1 dead under the main cookline make table upper shelf under the ticket unit. **Warning**
36-22-4    Basic - Floor area(s) covered with standing water by the rice cooker table. **Warning**
36-18-4    Basic - Floor/wall tiles cracked, broken or in disrepair. Flies present. **Warning**
08B-30-4    Basic - Food stored in dry storage area not covered. Open box of clear liquid fry oil on the lower table shelf. Open containers of spices and open bag of gram flour in the storage shed. **Warning**
36-14-4    Basic - Grease accumulated on kitchen floor and/or under cooking equipment **Warning**
36-24-5    Basic - Hole in or other damage to wall at the hand sink by the ice machine. The sink is no longer mounted completely to the wall with the hardware hole exposed. Holes in wall from shelving hardware in several kitchen areas. **Warning**

35B-05-4    Basic - Outer openings not protected during operation and vermin and/or environmental cross contamination present. Food storage shed door is observed open at the start of the inspection. Rodent droppings, live flies, live ants observed in the shed. **Warning**
29-20-5    Basic - Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Next to the ware wash line. Water is observed pooled in the hand sink. **Warning**
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. **Warning**
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. 2 live flies in the cookline area. Flies in food storage shed, qty 3 observed. **Warning**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Melted butter at cookline 81°F **Warning**
35A-09-4    High Priority - Presence of a large swarm of flies at the dumpster. Live ants observed on the food storage shed table next to open food packages. **Repeat Violation** **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 1 live roach with egg sac attached observed crawling on the wall above the hand sink by the ice machine. 11 live roaches observed on the soda boxes in the kitchen. Manager on duty is working to move the soda boxes and kill what is running. Inspector recommended disconnecting the boxes and taking them outside. 1 observed crawling on the wall behind the soda machine in the kitchen. 3 observed on the server tray rack stored next to the ice machine. 1 baby roach observed on the soda machine table crawling from under the ice tea unit. Two egg sacs observed under the ice tea unit. 10 live roaches, small and large, on the wall behind the kitchen table next to the prep line where baskets and glasses are stored. 1 live roach in open, partially used aluminum foil box on the prep counter by the fryer. 1 observed running across the floor from the cookline wheeled rack. Observed 3 live roaches on wires and tubing hanging from center kitchen by the make table. 7 live roaches observed at the outlet over the ware wash room table. 3 lice roaches on the dish machine. Several were killed during the inspection. **Warning**
35A-23-4    High Priority - Roach excrement and/or droppings present along the table top edge of the wall near the fryer. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 3 dried rodent droppings on the storage shed table next to open food packages/containers. Approximately 40 mixed soft and dried droppings on the food storage shed floor. 1 dried dropping on uncovered food take out container in the storage shed. 2 dried droppings in a metal pan hilding dried peppers in the storage shed. **Warning**
01B-03-4    High Priority - Stop Sale issued due to adulteration of food product. Loose dried peppers observed on top of cans on can rack in shed where rodent droppings are also observed on the cans. **Warning**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Open gram flour bag and a tray of open spice bowls in the food storage shed next to live ants, live flies and dried rodent droppings. One onion, one potato on the floor if the storage shed with chew marks, rotted areas. One bag of dried peppers in the storage shed with multiple holes. Unsure if holes are from human handling or chew marks. Bag is in the pan where droppings are observed. **Warning**
22-05-4    Intermediate - Cutting board(s) stained/soiled. (Multiple at make table units.) **Warning**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Observed a basket in the warewash line hand sink. **Warning**
