Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-18-4    Basic - Floor tiles cracked, broken or in disrepair. In kitchen. **Repeat Violation**
36-12-4    Basic - Floors not constructed to be easily cleanable. Floors in freezer room. **Repeat Violation**
08B-38-4    Basic - Food stored on floor. Container of oil on floor in kitchen. . Cases of food on floor in dry storage room.
36-24-5    Basic - Hole in or other damage to wall. Hole in wall around drain pipe under the triple sink. Rub marks present near opening. **Repeat Violation**
22-08-4    Basic - Interior of oven has heavy accumulation of black substance/grease/food debris.
29-19-4    Basic - Standing water in floor drain/floor drain draining very slowly. Floor drain in kitchen slow draining.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 4 live roaches found on shelf above hand wash sink in kitchen next to spices. 2 live roaches found behind shelving at buffet service area. **Repeat Violation**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 20 dry droppings found under push cart in kitchen. Two packages of Ramen noodles which have had the packages chewed through under the same cart. 10 dry dropping found in box of to go BBQ sauces. The operator discarded the box. 10 dry droppings found in utensil container on drying rack near triple sink. Too numerous to count dry rodent dropping under cabinet at dining area near toasters. Too numerous to count dry rodent droppings found on floor near dish ware storage rack next to triple sink.
35A-07-4    High Priority - Small flying insects in bar area. 5 flies in dining area near microwave. 5 flies under three compartment sink. **Repeat Violation**
29-37-4    High Priority - Spray hose at dish sink lower than flood rim of triple sink.
22-18-4    Intermediate - Clean utensil storage containers on drying rack near triple sink soiled. Wash rinse and sanitize dish-ware before use.
22-22-4    Intermediate - Encrusted material on can opener blade. **Repeat Violation**
16-35-4    Intermediate - No chlorine chemical test kit provided when using chlorine sanitizer at three-compartment sink/warewashing machine. **Repeat Violation**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. In kitchen. **Repeat Violation**
31B-03-4    Intermediate - No soap provided at handwash sink. Near buffet station in dining area. **Repeat Violation**
02C-04-4    Intermediate - Operator is not properly tracking/marking the number of days ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Ham in walk in cooler.
22-16-4    Basic - Intermediate - Walk-in cooler shelves soiled with food debris.
