Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-24-5    Basic - Stainless steel sheets rolling back away from wall. At time of inspection not caulked/sealed to wall.
36-27-5    Basic - Wall soiled with grease and debris on and around PVC pipe under hand sink in kitchen beside microwave.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. On cooks line.
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. cheese 51°F , lettuce 49°F , cubed tomatoes 49°F , sliced tomatoes 48°F , shredded lettuce 55°F , chicken 51°F. In deli prep top and reachin in cooler bottom of same unit on cooks line salad cooler.
03G-11-4    High Priority - Reduced oxygen packaged fish packaged onsite no longer frozen and not removed from reduced oxygen package. In prep area also in walk in cooler tuna steak filet. Package clearly marked to open when thawing.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Approximately 35 live roaches. 32 beside batter table and behind wall in corner of cooks line in kitchen. 3 more behind stainless steel panel. Behind flat top grill on cooks line.
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. On outside mop sink. In dish area hose bib near 3 compartment needs vacuum breaker.
02C-03-4    Intermediate - Commercially processed ready-to-eat, potentially hazardous (time/temperature control for safety) food opened and held more than 24 hours not properly date marked after opening. Milk in walk in cooler opened not date marked.
12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. 2 employees washed hands in prep sink.
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink.at hand wash sink on soda coffee prep.
31B-03-4    Intermediate - No soap provided at handwash sink. In prep area.
22-01-4    Intermediate - Soda gun soiled. In front bar.
16-52-4    Intermediate - Wash solution in three-compartment sink less than 110 degrees Fahrenheit. In 3 compartment sink in large bar area.
