Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. 2 dead behind mens room cabinet in stall. 1 at floor board at stall entrance. 1 dead under women's rest room cabinet. 1 dead on a dining room wine bottle display shelf. Operator swept up. **Warning**
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. (Side exit doors from the dining room.) **Warning**
36-31-4    Basic - Hole in ceiling. Above the alto sham unit where pipes run thru. **Repeat Violation** **Warning**
36-24-5    Basic - Hole in or other damage to wall. Holes in electronics closet where wires run thru and rodent droppings are observed. Holes also in alarm system cabinet where wires run thru. **Warning**

10-12-4    Basic - In-use ice scoop stored on soiled surface between uses. **Warning**
36-70-4    Basic - Objectionable odor in the dry storage room at the food shelving. **Warning**
16-46-4    Basic - Old labels stuck to food containers after cleaning. Observed a pan on the upper shelf over the three bay sink with a use first sticker. **Warning**

01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Qty 2: 4lb2.5oz can of skipjack tuna **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 2 dried droppings on the equipment storage lower shelf at the food prep area. 2 dried droppings on the floor behind the cookline stream table. 4 dried droppings on top of the Alto sham oven. Approximately 30 dried on the floor along the wall under the dry storage shelves and survellience/printer/electronic shelves. 12 dried droppings observed on multiple shelves if the electronic closet. 2 dried droppings on sanitary glove boxes. 1 soft dropping on a box of paper towels. Manager cleaned up the dropping. 5 droppings on top shelf boxes. Unable to reach. 10 dried droppings observed in aprons on bottom dry storage shelf. 3 dried droppings in alarm system cabinet in kitchen next to reach in freezer. 12 dried droppings on multiple food cans in the dry storage room. Manager discarded dropping. Approximately 20 dried droppings inside and open box of single service lids on the dry storage shelves. Box of lids discarded.aprons taken out of kitchen. Approximately 25 dried droppings on the floor of the chemical closet. 9 dried droppings on the dish machine. 2 dried droppings on the floor by the mop sink. 14 dried rodent droppings on the floor below the unused small ice bin in the room with the large ice machine. **Repeat Violation** **Repeat Violation** **Warning**
35A-21-4    High Priority - Rodent burrow or rodent nesting materials present in the box of single service lids. The manager discarded the box. Lid bags and the cardboard box were observed chewed. Chew marks on sanitary glove box. Entire end of the box chewed open. Box discarded. Loose aprons on bottom shelf of dry storage have loose dry pasta and droppings inside the apron layers. two dry pasta packages on shelf above are severely chewed into. 3 boxes of raisins chewed into. Boxes discarded. 10 boxes of powdered sugar, one box with an entire side chewed open. All discarded. One box of dry rice chewed into. Box discarded. Open bulk tea bag dispersed behind the ice machine where the area is wet. Tea is swollen in piles. Silver packaging lying in the area ID'd as tea. Lea and perrins sauce bottle package covered in plastic wrap chewed up with bread debris and dried droppings inside the box among the bottles. **Warning**
35A-18-4    High Priority - Rodent rub marks present along walls/ceilings of the back of a food can storage shelf. Also on walls along dry storage shelf behind dry pasta. **Repeat Violation** **Warning**
01B-03-4    High Priority - Stop Sale issued due to adulteration of food product. Two large bags of pasta have chew marks. Manager discarded pasta. 3 boxes of raisins. Boxes discarded. 10 boxes of powdered sugar. Dry rice box Pepper corn bottles, lids chewed **Warning**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. 6 salt packages where the top surface at the spout area is brown and yellow. Open bulk tea bag dispersed behind the ice machine where the area is wet. Tea is swollen in piles. Silver packaging lying in the area ID'd as tea. **Warning**
22-57-4    High Priority - Thermotape failed to turn black to indicate the sanitization temperature of 160 degrees Fahrenheit was achieved on the dish surface. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired. Several cycles, tried two different test strips. **Warning**
35A-09-4    High Priority - Two dead beetles observed in the dining room. **Corrected On-Site** **Warning**
