Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. 2 dead roaches on floor near triple sink. 1 dead roach on shelf with clean dish ware next to triple sink. 1 dead roach on wall above hand wash sink next to triple sink.
12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils.
36-73-4    Basic - Floor soiled/has accumulation of debris. Near triple sink.
14-66-4    Basic - Floor-mounted equipment that is not easily movable is not sealed to the floor and has legs that provide less than a 6-inch clearance between the floor and equipment. Storage shelf near mop sink no 6 inch clearance from floor.
08B-38-4    Basic - Food stored on floor. Plastic container of sauce in the walk in cooler. **Repeat Violation**
36-14-4    Basic - Grease accumulated on kitchen floor and/or under cooking equipment
14-70-4    Basic - Ice buildup in walk-in freezer.
14-31-4    Basic - Nonfood-grade bags used in direct contact with food. To go bags used to hold raw meats in the walk in freezer. **Repeat Violation**
33-16-4    Basic - Open dumpster lid.
06-04-4    Basic - Potentially hazardous (time/temperature control for safety) food thawed at room temperature. Beef thawing at room temperature. The operator moved to a prep sink and placed under cool running water. **Corrected On-Site**
25-05-4    Basic - Single-service articles improperly stored. Boxes of to go items on floor in storage area in back kitchen. **Repeat Violation**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Bulk container of sugar no labeled. **Repeat Violation**
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Small flies found in dish area.
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Peanut sauce 45°F on buffet. I discussed with the operator how to properly hold food items on ice. Curry sauce homemade on crash cart 47°F, garlic and oil mixture 46°F on crash cart. I had the operator put these items on ice. **Repeat Violation**
03B-01-4    High Priority - Potentially hazardous (time/temperature control for safety) hot held at less than 135 degrees Fahrenheit or above. Tofu 120°F, egg rolls 100°F, fried tofu 111°F, the operator added lids so the items would stay a minimum of 135°F **Corrective Action Taken**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 1 live roach on wall in kitchen near hand wash sink. 5 live roaches on wall above two bay prep sink. 1 live roach in Saran Wrap box on prep table. 3 live roaches in box of aluminum foil under prep table in kitchen. 2 live roaches under prep table in kitchen. 3 live roaches under an egg carton on shelf of prep table next to ice machine. 1 live roach under a stack of napkins at front counter area.
12A-09-4    High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Employee rinsed gloves off in prep sink. I had the R,played wash hands and change gloves. We discussed the proper hand washing procedures.
22-20-4    Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine.
22-24-4    Intermediate - Accumulation of black/green mold-like substance on/around soda dispensing nozzles at self serve station.
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Near triple sink and walk in cooler. **Corrected On-Site** **Repeat Violation**
31B-03-4    Intermediate - No soap provided at handwash sink. Hand wash sink near walk in cooler. **Corrected On-Site** **Repeat Violation**
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Cooked sauces in the walk in cooler. **Repeat Violation**
22-06-4    Intermediate - Slicer blade soiled with old food debris. Hand held slicer on top each near triple sink. **Repeat Violation**
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Spray bottle on floor near soda boxes not labeled.
