Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. 1 dead roach in the foil box on the slicer. 1 on sauce shelf next to make table under sauce bottle. Two roach eggs in the foil box. **Warning**

36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Behind 3 bay and hand sink. Pipes, window sill, window security bars, shelving. **Warning**
16-25-4    Basic - Warewashing conducted in outdoor area. French fry slicer mounted on a wheeled floor stand. **Warning**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Multiple bulk containers of white product below the microwave. **Warning**
22-45-4    High Priority - Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. French fry slicer mounted on a rolling floor stand that is used outside under the covered area. Unit was rinsed outside with a hose. Discussed proper cleaning methods and location of food equipment. Clean in place is acceptable for wash, rinse, sanitize,but not out in the yard with the hose outside. **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 2 live roaches observed on prep counter when plastic wrap box was moved to be inspected. Sub sandwiches were just prepped at this counter. One live observed by moving antennae on underside of the same table at a mounted disc shaped trap. 20 small live roaches clustered on the floor in a grout line between the back of the make table and the microwave/prep table. Employee poured undilluted bleach on the cluster. 1 live roach in aluminum foil box on bottom shelf of slicer table. 1 live roaches on the wall under the bar hand sink. 2 live roaches in corner above hand sink by hanging certificates in plastic covers. **Warning**
35A-23-4    High Priority - Roach excrement and/or droppings present. Underside of prep table by traps. Also in large area on wall near the door to the front of the house where a sign once hung, observed by paint differences. **Warning**
