Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-22-4    Basic - Floor area(s) covered with standing water under the loose tiles by the pizza oven and near the stove under a floor mat. **Warning**
36-18-4    Basic - Floor tiles cracked, broken or in disrepair. Large area in front of the pizza oven. Approximately 12 tiles missing from the pizza oven area. Also a large area by the back door next to a cooler. Also near the stove under a floor mat. **Warning**

08A-18-5    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature in reach-in freezer - all products not commercially packaged. Raw beef over breaded vegetables in upright freezer. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 50+ dried droppings along two walls behind the ice machine and along the ice machine wall and an adjacent wall behind multiple freezers and coolers. The area is tight fitting to get an accurate count behind the units. The 50 counted were accessible. 2 dried droppings on slicer surface under the sliding attachment. Supervisor took slicer apart for cleaning. 20 dried droppings on the top and at the base of the water heater. 4 dried droppings behind the floor mixer. 60+ droppings, too many to count, along entire dining room bar area. 5 observed semi-soft on a box under the bar three bay sink. 4 dried droppings in small dining room to the left of the front door. Droppings observed behind the coke machine in the center dining room, unable to count. 4 dried droppings under the women's restroom sink. . **Repeat Violation** **Warning**

12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. Observed employee wash/rinse hands in center bay of three bay sink. The hand sink is at the front line near the register. **Warning**
31A-07-4    Intermediate - Handwash sink missing in multiple food preparation areas. The cookline hand sink is up around the front line by the register toward the bar. The cook was observed washing hands in the three bay sink. Also, in the adjacent room by the employee bathroom Are a floor mixer, a slicer, and a small make table. . **Warning**
22-28-4    Intermediate - Interior of reach-in cooler soiled with accumulation of food residue. Upright freezer near ice machine. **Warning**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink at the cookline. Paper towels put near the cookline hand sink. No paper towels at the register area hand sink. **Corrective Action Taken** **Warning**
