A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom.

Basic - Dead roaches on premises.
Observed approximately:
- 7 dead roaches underneath prep table on cook line.
- 10 dead roaches underneath heater unit next to white chest freezer.
- 1 dead roach on a plate by the cook line.
- 5 dead roaches on the wall, behind reach in cooler on the cook line.
- 2 dead roaches on wait station area where ice machine #2 is stored.
- 2 dead on top of dish washer. **Warning**
Basic - No handwashing sign provided at a hand sink used by food employees.
- observed no hand wash signs on hand wash stations located by the back door and cook line. Operator was provided with hand wash signs. **Corrected On-Site** **Warning**
Basic - Water leaking from pipe and/or faucet/handle.
- observed leaking pipe on hand wash sink, located on the cook line.
- observed leaking pipe on 3 compartment sink. **Warning**
High Priority - Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly.
- Observed dish machine not sanitizing properly, operator ran the machine 4 times and the reading for chlorine was 10ppm. Operator advised to set up 3 compartment sink, until dish machine is properly repaired. **Corrective Action Taken** **Warning**
High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit.
- Observed approximately 8lbs of baked potatoes on cooling rack at a range temperature of 75°-80°F, per operator baked potatoes were left overnight, operator discarded products.Stop sale was issued. **Warning**
High Priority - Roach activity present as evidenced by live roaches found.
Observed approximately :
- 3 live roaches underneath dish washer area on kitchen
- 7 live roaches underneath stove and grill on cook line
- 3 live roaches underneath prep table on the cook line **Repeat Violation** **Warning**
High Priority - Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse.
- Baked potatoes. **Warning**
High Priority - Toxic substance/chemical stored by or with food.
- Observed bottle of bleach, being stored in the dry storage area next to dressings and single service items. Operator removed bleach. **Corrected On-Site** **Warning**
Intermediate - Hot water not provided/shut off at employee handwash sink.
- observed no hot water at hand wash sink on the cook line. Operator turn on hot water. Hot water reaching 120°F. **Corrected On-Site** **Warning**
Intermediate - No paper towels or mechanical hand drying device provided at handwash sink.
- Observed no paper towel on hand wash sink on the cook line, operator provided paper towels. **Corrected On-Site** **Warning**
