Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-01-4    Basic - Floor not cleaned when the least amount of food is exposed.. Floors underneath bar glass washer has accumulation of lids, cups and paper towels on floor. Floor behind cash register at main outside bar has accumulation oh cups and lids underneath on floor. **Warning**
36-11-4    Basic - Floors not maintained smooth and durable. Standing pockets of water by ovens and backdoor. Flatbread area. **Warning**
08B-32-4    Basic - Food being prepared outside. Observed employee prepping flatbread pizza at make table outside door of kitchen. Area is a covered walkway where soda boxes, chest freezer, make table, short freezer, upright cooler holding dry storage and a double oven are used for food prep. Cannot take place here as this area is not a fully enclosed room protected from pests and environmental contamination. **Warning**

33-31-5    Basic - Unnecessary items/unused equipment on the premises. Stacks of pallets and wood next to dumpster. **Warning**
36-02-5    Basic - Unsealed concrete floor in food prep area , flat bread area, under cooler and ovens. **Warning**
25-02-4    Basic - Unwrapped single-service utensils not presented so that only the handles are touched. Black sip straws stored in box at street bar. Single service spoons stored above refrigerator not covered. **Warning**
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Two flies observed on freezer next to flatbread prep table where bulk cheese is held. **Warning**
35A-21-4    High Priority - Many traps in condiment cabinet. 2 traps have chewed sugar packets in them at end of main outside bar around corner. Sunflower/pumpkin seeds at front bar by drain chase. **Warning**
35A-09-4    High Priority - Presence of insects, rodents, or other pests. Live ants observed crawling on coolers and other equipment outside the kitchen past the flatbread cooler/oven area. Live white insects in bar beer bottle bucket. Appear to be maggots. Approximately 20. **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 2 live roaches in single service storage behind middle outside bar. Observed open lid packages lying on the droppings. Observed 3 egg casings. Observed approximately 30 live roaches behind white chest cooler next to walk in cooler. Roaches also on pallet under chest cooler and food oil container next to cooler. Chest cooler is located outside of cooking line , approximately 5 feet past the doorway. Observed 2 live roaches under upright cooler used for dry storage. Cooler is next to flatbread freezer outside kitchen. 1 live roach behind double stack oven. **Warning**
35A-23-4    High Priority - Roach excrement and/or droppings present. Too many to count in single service cabinet behind middle bar. Dropping are soft. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed 38 hard dropping in middle condiment cabinet. Observed 19 hard droppings in first condiment cabinet. Salt and pepper grinders in cabinet with single service napkin holders in these cabinets. One dropping observed in pepper shaker top. One fuzzy dropping in wet wipe container attached to wet wipes. Location of these cabinets is at end of outside main bar around corner. Observed 1 dropping on a wrapped single service silverware in middle condiment cabinet. **Warning**
35A-07-4    High Priority - Small flying insects in bar area at beer tap drain in floor at main outside bar. Approximately 10, small swarm observed. **Warning**
01B-03-4    High Priority - Stop Sale issued due to adulteration of food product. Salt and pepper grinder units. Held in unit with droppings. On shaker end soiled with dropping debris. **Warning**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Ice filled in the morning in street bar covered with non food grade plastic and wood cover. Operator discarded and sanitized ice bins **Corrected On-Site** **Warning**
22-02-4    Intermediate - Accumulation of food debris on food-contact surface. Beer buckets stored behind bar had standing water in them. One soiled bucket had approximately 15 small bugs in the bottom. **Warning**

22-22-4    Intermediate - Encrusted material on can opener blade. In dish wash room, mounted to table. **Warning**
22-28-4    Intermediate - Interior of reach-in cooler soiled with accumulation of food residue. Flat bread cooler and multiple beer coolers. **Warning**
51-16-5    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans, plan review application and payment for plan review to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Hallway outside main kitchen is covered but unsealed. Area has exposed, dusty pipes above ovens and food prep coolers. 4 chest freezers, two-2 door upright coolers, a double oven, 2-single door coolers, a low freezer and a pizza make table. **Warning**
