A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Building components, attachments or fixtures in poor repair- interior bar counter has section with rotted wood, not smooth and easily cleanable.
Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers- over ice machine. **Repeat Violation**
Basic - Perimeter walls and roofs do not effectively protect establishment against environmental cross contamination or the entrance of pests- ice machines room. **Repeat Violation**
High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit- sausage 53, meatballs 51, ham 58. Items in cooler since yesterday. Discarded. **Corrective Action Taken**
High Priority - Roach activity present as evidenced by live roaches found in kitchen- 4 live roaches on wall by the Everest reach in cooler, approximately 15 live roaches on floor by food preparation cooler (left side of room corner/hood system), approximately 40 live roaches alongside floor edge under clean utensils rack/food preparation sink.
Intermediate - No plan review submitted and approved - renovations were made or are in progress. Waitststion added to patio room, outside bar has been moved and remodeled. Must submit plans, plan review application and payment for plan review to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011.

Intermediate - Pizza prep reach in cooler not maintained in good repair. Do not store potentially hazardous (time/temperature control for safety) food in this unit until the unit is repaired. Ambient temperature ~ 51°
Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked- taco beef prepared 10/9/16 not date marked.
