Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Cook on cook line and cook frying tortilla chips no hair restraint.
14-11-4    Basic - Equipment in poor repair. Bar coolers rusted and pitted above doors.
36-17-4    Basic - Floor tiles missing. Multiple in walk-in cooler at walk-in freezer door. On cook line missing tiles.
36-24-5    Basic - Hole in or other damage to wall. In hot water heater room, damaged and mold-like substance.
05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. Bar reach-in cooler no ambient thermometer. Cook line make table ambient air thermometer broken. Thermometer reading 55°F.
14-31-4    Basic - Nonfood-grade bags used in direct contact with food. Tortillas on cook line store in Togo bag.
29-49-5    Basic - Observed standing water in bottom of reach-in cooler. Cook line reach-in cooler, bottom of cooler.
33-16-4    Basic - Open dumpster lid. Grease and trash dumpster open. Drain plug missing
23-10-4    Basic - Walk-in cooler and/or walk-in freezer gaskets soiled with slimy/mold-like build-up. Walk-in cooler and cook line reach-in coolers.
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Three compartment sink in dish area drain pipe leaking. Hand wash sink in dish area has no drain connected. Hand wash sink in server area faucet continually drips. **Repeat Violation**
12A-17-4    High Priority - Employee rubbed hands together for less than 10-15 seconds while washing hands. Cook worked with raw foods and washed hand for about five seconds.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Five flys in kitchen.
01D-01-4    High Priority - Nonexempt fish offered raw or undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded. Ceviche served with talipa, no parasite records on site.
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. On cook line make table, beef 44°F, chicken 46°F. Salsa 46. Manager instructed employee to place ice on product. Retemped beef at 42°F, chicken at 40°F after one hour. **Corrective Action Taken**
08A-17-5    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature in walk-in freezer - all products not commercially packaged. Walk-in freezer: Beef over shrimp, Manager moved at time of inspection. Walk-in cooler Turkey thawing over pork. Manager moved turkey at time of inspection. **Corrected On-Site** **Repeat Violation**
12A-09-4    High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Employee placed raw meat in pan and did not change gloves or wash hands.
41-11-4    High Priority - Toxic substance/chemical stored by or with single-service items. Floor cleaner stored next to Togo boxes and portion cups. Degreaser bottle on shelf of prep table. Soap over clean dishes in store room. Manager moved degreaser at time of inspection. **Corrective Action Taken**
29-42-4    High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Missing on both hoses
22-05-4    Intermediate - Cutting board(s) stained/soiled. Make table on cook line
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. At ice machine, blocked by construction equipment. Manager moved at time of inspection. **Corrected On-Site**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Hand wash sink at prep area and next to ice machine. Sign missing at bar hand wash sink. Manager filled towels and placed sign at time of inspection. **Corrected On-Site** **Repeat Violation**
