A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Attached equipment soiled with accumulated dust, grease, or food debris. Pipes are greasy, dusty at ansul box and air handler/ducts. **Warning**
Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance.
Dust also on pipes running along walls.

Basic - Dead roaches on premises. 1 in the dining room by the coupon rack. 4 dead under the wire rack shelf just inside the kitchen where delivery bags are.
23 dead around the soda cooler and back of pizza make table cooler near the front line.
2 dead in ladies restroom floor.
2 dead in the pizza make table reach in cooler.
4 dead behind the microwave shelf.
1 dead and one egg sac on the wood counter for the computer.
3 dead directly under microwave, 1 inside base of microwave viewed thru a vent hole.
3 dead under the table at the pizza prep station.
Too many to count piled in an outlet box behind the soda cooler/pizza side table.
18 dead, 1 with an egg sac, on the pizza make table counter under the sheet metal top. The sheet metal was not attached. It has a black flex-plastic sheeting on the unsealed wood surface of the pizza make table.
4 dead on the floor between the upright freezer and the wall.
1 dead on the mixer. 1 dead, smashed on the side of the upright freezer.
Too many dead to count at the water heater, and tray.

Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. In walk in and over table and food prep area. **Warning**
Basic - Nonfood-contact equipment exposed to splash/spillage not constructed of materials that are corrosion resistant, non-absorbent, smooth and easily cleanable.
Ryobi drill with mixing head. **Warning**
Basic - Single-service articles not stored inverted or protected from contamination.
Unwrapped single service take out containers in two opened boxes under the pizza prep side table. **Warning**
High Priority - Roach activity present as evidenced by live roaches found.
1 under the front counter, on its back.
6 live on the floor at the front counter in the kitchen. 3 in an open box of take out bags in the kitchen. Box was removed from the premise.
4 on the yellow wall by the register area/ wood counter area.
1 live on the wood counter computer area.
1 live on the floor under the pizza side table.
1live observed running into the open outlet box behind the soda cooler/behind the pizza side table.
1 live by soda rack.
1 live on the sanitizer
2 live on the pizza make table.
1 live on the gasket of the upright freezer by the 3 bay.
3 live on the sanitizer supply over the 3 bay sink.
1 live on lower table by canned olives under the black vinyl cover **Repeat Violation** **Warning**
High Priority - Roach excrement and/or droppings present on wall pipes next to the soda machine where the bracket holds the pipe to the wall. 1 live roach nearby on the wall.
Heavy roach droppings on the upright freezer gasket by the 3 bay sink.
Heavy Roach droppings on the above-sink chemical bottle racks. **Warning**
Intermediate - Accumulation of food debris/grease on food-contact surface. Uncooked pizza stack racks on pizza prep side table are soiled with sauce. **Repeat Violation** **Warning**
