Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.

3 live roaches crawling inside and under motor/ compressor o flip top cooler close to dish area on cook line.
2 live roaches on wall, crawling behind crevices in cove molding between stove and single door refrigerator at cook line.
1 live roach crossing over from cook line to 2 door stainless refrigerator next to,hand wash sink for dish area.
3 live roaches on floor in storage area at kitchen exit to bar.
2 live roaches crawling up the wall into ceiling tiles, in storage are at kitchen exit to bar.
Intermediate - Reach-in cooler not maintained in good repair. Do not store potentially hazardous (time/temperature control for safety) food in this unit until the unit is repaired.
Both flip top cooler at cook line 50°F -52°F ambient temperature.
Both cooler were emptied.
