A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

01B-07-4    High Priority - Food with mold-like growth. See stop sale. In walk in cooler: 1 Case of celery stalk(32ct) 1 Qt of opened sour cream(dated for expiration 11/16) 3 heads of red cabbage. **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed approx 20 live roaches in reach in cooler make table on cookline. Observed approx 50 live roaches under chest freezer on cookline. Observed 2 live roaches at server station. Observed approx 10 live roaches under trash bin in front of hand washing sink on cookline. Observed 2 live roaches behind poster on wall at pass through from server area to kitchen. Observed 3 live roaches in cabinet style hand washing sink on cookline. **Warning**
35A-23-4    High Priority - Roach excrement and/or droppings present. Observed roach excrement along hinges of cabinets. Observed roach excrement along wooden shelving units. Observed roach excrement behind posters attached to wall in kitchen. Observed roach excrement at brackets of storage shelf above three compartment sink. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approx 50 hard, dry rodent droppings in dry storage room(on stand mixer, on plastic food storage bins, under fryer filter, along baseboards) Observed approx 25 hard, dry rodent droppings behind cooking equipment on cookline. Observed approx 20 hard, dry rodent dropping along walls of dining room. **Warning**
28-26-4    High Priority - Sewage/Wastewater observed backing up through floor drains in dish area and end of cookline when dish machine and hand washing sink is turned on. Water is unable to be cleaned up before more water is backed up. Water is pooling in noted area. **Warning**
12A-09-4    High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Cook handled soiled equipment, cracked raw eggs, and failed to change gloves when handling ready to eat foods.
