Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

41-07-4    High Priority - Container of medicine improperly stored. Multiple containers of medicine(pills, peroxide) stored on make-table on cookline. **Warning**
03D-01-4    High Priority - Cooked potentially hazardous (time/temperature control for safety) food not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within 2 hours. Yogurt(temperature taken at 1:45p-90F) was made at 6am. Per cook, yogurt has been cooling at room temperature(under prep table on cookline) since product was made at 6am. See stop sale. **Warning**
12A-07-4    High Priority - Employees failed to change single-use gloves when walking out of establishment and returning into establishment to work with food/clean utensils. **Warning**
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed approx 25 live small flying insects in kitchen, on food preparation tables, food storage bins and clean utensils. **Warning**

08A-22-4    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature in reach-in cooler. Raw chicken being stored over raw shrimp in make-table unit on cookline. **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed approx 20 live roaches on dish machine. Observed approx 20 live roaches on interior of non-working reach in cooler being utilized for dry goods. Observed approx 25 live roaches on utensils stored between equipment on floor. Observed 3 live roaches on interior of door and gasket of make-table on cookline. Observed approx 10 live roaches on wheel well of make-table on cookline. Observed approx 50 live roaches on bottom shelf of prep table on bags of bulk dry goods(flour, cornstarch). Observed approx 100 live roaches on bottom side prep tables placed together in kitchen; active food preparation on tables observed. Observed approx 10 live roaches on can opener attach to prep table in kitchen. Observed 2 live roaches in bulk sugar container under prep table in kitchen. **Warning**
12A-09-4    High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Cook handling raw chicken with single-use gloves and then continuing to handle ready to eat foods with the same single-use gloves. **Warning**
