A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Opened employee beverage container in a cold holding ( walkin cooler) unit with food to be served to customers.

Basic - Working containers of food removed from original container not identified by common name.
Observed flour bin in dry storage and sugar in dry storage not properly labeled. **Warning**
High Priority - Approximately 8 live, small flying insects in prep area, 12 in dishwasher area, 10 live in dry storage area, and 14 in front service counter.
Observed live small flying insects on boiled eggs, and raw beef in triple sink compartment. **Warning**
High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit.
Observed raw beef from 51°F to 58°F, raw fish at 56°F, and raw turkey 70°F, on floor in kitchen. Foods has being stored on floors for over an hour waiting to be clean.

High Priority - Raw animal food stored over ready-to-eat food in walk-in cooler.
Observed raw shell eggs over cooked pork.

High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature in reach-in freezer.
Observed raw chicken mixed together with raw beef in back door chest freezer. **Warning**
High Priority - Toxic substance/chemical stored by or with food.
Medicine above chest freezer in front service area and above prep table at cookline.
Employee removed medicine from both areas. **Corrected On-Site** **Repeat Violation** **Warning**
Intermediate - Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection.
Observed cooked rice from 52°F to 54° F within 2 hour, cooked beef from 49°F to 53°F within 2 hour, cooked chicken from 52°F to 57°F within 2 hour, cooked pork fom 52°F to 57° F with in two hours in walk in cooler. Deliveries being put away , doors left open for long period of time. Doors continuously being opened and closed. Operator to leave doors closed until unit can adjust temperature to 41° F or below and foods can cool properly. **Corrective Action Taken** **Warning**
Intermediate - Interior of reach-in freezer soiled with accumulation of food residue.
Observed back door chest freezer has accumulation of meat blood. **Warning**
Intermediate - No paper towels or mechanical hand drying device provided at women handwash sink.
