A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. 60 dead flies in the kitchen dead on a sticky tape fly trap.
Basic - Floor soiled/has accumulation of debris. Floor under and behind keg coolers has buildup of dust, dirt and debris. Floor in kitchen under 3 compartment has buildup of debris.
Basic - Hole in or other damage to wall. 2 holes in wall with insulation showing under handsink in kitchen. Wall in men's restroom has 2 holes with insulation coming out.
High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. 30 live flies in kitchen area under prep counter near grease trap. 10 live flies next to ice machine. 6 live flies in men's restroom. **Warning**
High Priority - Rodent activity present as evidenced by rodent droppings found. 1 hard rodent dropping on stainless steel prep table in kitchen next to microwave, 2 soft rodent droppings under 3 compartment sink in kitchen 3 hard rodent droppings under 3 compartment sink in kitchen. 4 hard rodent droppings under the big screen tv in the bar. 2 hard rodent droppings under the keg cooler near wall in bar. 2 hard rodent droppings near wall in pool table room. 2 hard rodent droppings in storage area near ice machine. 2 dry rodent droppings in hole in men's restroom wall. **Warning**
High Priority - Spray bottle with chemical/toxic substance stored near/on/above food preparation surface or food. Spray bottle of shout next to microwave.
Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine. Ice machine interior black moldlike debris. **Repeat Violation**
Intermediate - No chlorine chemical test kit provided when using chlorine sanitizer at three-compartment sink/warewashing machine. No chlorine test strips.
