A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - - From inspection on 2017-08-25: Basic - Floor area(s) covered with standing water.
Observed in front of the dish machine. Water is leaking from ceiling in dishwashing area. Water is leaking from wall baseboard behind walk in freezer and bag in box soda, standing water on floor is observed in this area. **Repeat Violation** **Warning** - From follow-up inspection on 2017-08-26: **Time Extended**
Basic - - From inspection on 2017-08-25: Basic - Food stored in a location that is exposed to splash.
Observed with packaged burger buns on storage crates in ware washing area. Water splashes from ceiling onto shelves next to dish machine and splashes onto buns, water droplets observed on plastic packaging containing buns.
Manager discarded buns and moved them to a location away from splash.
Also observed water from screw in ceiling in walk in cooler leaking onto sheet pans and pre cooked chili, about 10lbs. Containers of chili are standing in splash water and Saran Wrap covering them have pooled water on top, see stop sale. Manager removed chili containers. Manager instructed to move rolling cart with foods in walk in cooler away from the leaking in this cooler.
Clean plates come out of this area where leaking is observed.

Leaking in ware washing area, in front of dish machine.
It is also observed in walkway between milkshake station and ware washing area.
It is also leaking into black plastic tray from ceiling in corner of kitchen near Icecream freezer. Water is leaking from wall baseboard behind walk in freezer and bag in box soda.
Also observed water from screw in ceiling in walk in cooler leaking onto sheet pans and pre cooked chili, about 10lbs. Containers of chili are standing in splash water and Saran Wrap covering them have pooled water on top. Water is also leaking from the light in walk in cooler.
Water is also shooting up from metal baseboard on floor at walk in cooler whenever stepped on and water is settled here as well. **Repeat Violation** **Warning** - From follow-up inspection on 2017-08-26: **Time Extended**
High Priority - - From inspection on 2017-08-25: High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area.
Observed 8-12 flies flying in ware washing and milkshake area and 25 plus small flies on crates on drain board in ware washing area. **Repeat Violation** **Warning** - From follow-up inspection on 2017-08-26: **Time Extended**
High Priority - - From inspection on 2017-08-25: High Priority - Stop Sale issued due to adulteration of food product.
Water splashes from ceiling onto clean stacked pans on shelves next to dish machine and splashes onto buns, water droplets observed on plastic packaging containing buns. Stop sale issued on 1 package of buns, about 30 buns. Manager discarded buns at time of inspection.
Also observed water from screw in ceiling in walk in cooler leaking onto sheet pans and pre cooked chili, about 10lbs.
