Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

03D-01-4    High Priority - Cooked potentially hazardous (time/temperature control for safety) food not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within 2 hours. Ground beef in walk in cooler at 47°, cooked the night before. **Warning**
12A-16-4    High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed kitchen employee begin work without washing hands and did prep, handling raw beef.
09-23-4    High Priority - Employee touching ready-to-eat food with their bare hands - ready-to-eat food mixed with/added to raw animal food and ready-to-eat food did not reach minimum required cooking temperature for the raw animal food when the final product was cooked. Establishment has no approved Alternative Operating Procedure. Observed kitchen employee contacting raw ground beef with bare hands to portion amounts of food. **Warning**
35A-19-4    High Priority - Live rodent present. Observed 1 mouse in storage closet where tools are stored in bakery section of restaurant. **Warning**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Cooked ground beef cooked the night before at 47°. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Numerous dry droppings found in storage area of bakery section of restaurant. Observed 15 moist droppings in bakery storage section of restaurant. **Warning**
01B-02-4    High Priority - Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. Cooked ground beef at 47, cooked the night before. **Warning**
31A-13-4    Intermediate - Employee used handwash sink as a dump sink. Observed ice in hand wash sink located at front counter. **Warning**
