Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

24-06-4    Basic - Clean utensils or equipment stored in dirty drawer or rack. Clean plates and cups stored on dirty shelving with dead roaches.
35A-03-4    Basic - Dead roaches on premises. Observed approximately 1 dead roach in water well at steam table, 1 dead roach inside blender cup in storage shelving by steam table, 16 dead roaches in storage shelving by steam table, 1 dead roach inside red storage cooler, 1 dead roach on floor by round table at cookline, 2 dead roaches under cloth cover table in front counter area and 1 dead roach inside freezer in front counter area.
12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. **Repeat Violation**
36-62-4    Basic - Light not functioning in walkin cooler.
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Aluminum foils containers not properly inverted on side prep table .
23-05-4    Basic - Soil residue build-up on nonfood-contact surface. Piping under 3 compartment sink heavily soiled. Shelving unit by steam table soiled with dead roaches. Microwave exterior soiled, round table by cookline soiled. Table in front counter area soiled with dead roaches. Red storage cooler soiled with dead roaches inside.
22-45-4    High Priority - Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. Blender cup contaminated by dead roach inside it, on shelving by steam table.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed approximately 2 live roaches inside bean storage bin at cookline, 1 live roach inside container of salt at cookline, 3 live roaches under steam table at cookline, 4 live roaches on plates in storage shelving by steam table, 1 dead roach inside red storage cooler, 1 dead roach on floor by round table at cookline, 1 roach casing by pot on prep table at cookline, 20 live roaches under prep table at cookline, 9 live on loose paneling behind stove, 6 live roaches by 3 compartment sink, and 3 live roaches under cloth covered table in front counter area.
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Salt container with live roach inside it crawling over food and 2 live roaches inside bean storage bin at cookline.
22-16-4    Basic - Intermediate - Walk-in cooler shelves soiled with food debris.
