Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

23-07-4    Basic - Gaskets with slimy/mold-like build-up on white chest freezer across from triple sink. **Warning**
14-31-4    Basic - Nonfood-grade bags used in direct contact with food. Observed grocery store bags used in direct contact with wontons and cabbage in walk in cooler. **Repeat Violation** **Warning**
06-05-4    Basic - Potentially hazardous (time/temperature control for safety) food thawed in standing water. Operator is thawing raw shrimp at triple sink in standing water. **Warning**
08B-39-4    Basic - Raw vegetables not washed prior to preparation. Observed employee slicing unwashed white mushrooms at preparation table in rear of kitchen, mushrooms are still in cardboard box with dirt on them. Instructed operator to wash produce before preparation. **Warning**
29-20-5    Basic - Standing water or very slow draining water in handwash sink next to triple sink. **Repeat Violation** **Warning**
38-02-4    Basic - Twenty (20) foot-candles of light at least 30 inches from floor not provided in handwashing area, in employee restroom. **Warning**
01B-37-4    High Priority - Cooked/heated potentially hazardous (time/temperature control for safety) food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within six hours. See stop sale. Observed in white Frigidaire reach in cooler near front counter: White Frigidaire reach-in cooler:; sweet and sour chicken approximately 5lbs (52°F - Cooling); bourbon chicken approximately 0.5lbs (51°F - Cooling); General tso chicken approximately 2lbs (52°F - Cooling); egg rolls approximately 5lbs (51-53°F - Cooling). Foods were cooked and began cooling yesterday per operator.Ambient air temperature is 67°F in this cooler. **Warning**
12A-16-4    High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Cook began to handle clean pots in cookline after coming from outside and not washing hands. **Warning**
03D-05-4    High Priority - Potentially hazardous (time/temperature control for safety) food cooked/heated yesterday/on a previous day not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within a total of 6 hours. Food has been cooling overnight. See stop sale. Observed in white Frigidaire reach in cooler near front counter: White Frigidaire reach-in cooler:; sweet and sour chicken approximately 5lbs (52°F - Cooling); bourbon chicken approximately 0.5lbs (51°F - Cooling); General tso chicken approximately 2lbs (52°F - Cooling); egg rolls approximately 5lbs (51-53°F - Cooling). 03B-01-4    High Priority - Potentially hazardous (time/temperature control for safety) hot held at less than 135 degrees Fahrenheit or above. Observed in steam table: cooked pork wontons (118°F - Hot Holding), operator corrected it by reheated food to above 165°F. 08A-05-4    High Priority - Raw animal food stored over ready-to-eat food. Raw pork stored above ready to eat sauces in walk in cooler. **Warning**
08A-21-4    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature in walk-in cooler. Raw chicken stored above raw shrimp in walk in cooler. **Repeat Violation** **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed at least 40 dried rodent droppings along wall behind white reach in freezer and dry storage shelf with cans near water heater. 05-05-4    Intermediate - Ambient air thermometer in holding unit not accurate within plus or minus 3 degrees Fahrenheit. Observed in white Frigidaire reach in cooler near front counter, temperature reads 56°F, inspector's thermometer reads 67°F in the same area of this cooler. **Warning**
13-06-4    Intermediate - Employee with artificial nails/nail polish working with exposed food without wearing intact gloves. Observed female employee with painted artificial nails working in kitchen preparing foods. **Warning**
31A-03-4    Intermediate - Handwash sink at back door not accessible for employee use due to white plastic item stored in the sink. **Repeat Violation** **Warning**
27-16-4    Intermediate - Hot water not provided/shut off at employee handwash sink. Observed at handwash sink near back door. Employee corrected it. **Corrected On-Site** **Warning**
16-32-5    Intermediate - Incorrect chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink. Operator uses bleach and has quaternary ammonia test kit. **Warning**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink in restroom. **Warning**
05-08-4    Intermediate - No probe thermometer provided to measure temperature of food products. **Warning**
02C-04-4    Intermediate - Operator is not properly tracking/marking the number of days ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Observed with cooked ribs and chicken inside of white reach in freezer next to triple sink. Reviewed date marking with operator. **Warning**
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Observed with all prepared foods, cooked and fried chicken. **Repeat Violation** **Warning**
