Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-75-4    Basic - Build-up of soil/debris on the floor under shelving. Area behind the moveable cart in wait station, heavy debris .
36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair. Ceiling flaking near bottom of stairs and walk in freezer
21-15-4    Basic - Clean and soiled linens not stored separately from one another. Large amount of loose wiping cloths both clean and soiled under dish washing line, all removed by dish washer. **Corrective Action Taken**
24-24-4    Basic - Clean equipment and utensils stored in a room that is not fully enclosed (open to outdoors/screened). Equipment stored upstairs exposed to outdoors due to broken plexiglass window.
24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Glass plates in walk in cooler on top wire shelf not inverted.
12B-02-4    Basic - Employee beverage in a food preparation or other restricted area. Wait station and dish washing station, items moved. **Corrective Action Taken**
36-08-4    Basic - Floor and wall junctures not coved in food preparation/storage/warewashing areas or bathroom. Back dry stock room
36-71-4    Basic - Floor drains/drain covers heavily soiled. Thru location and in front of house.
36-01-4    Basic - Floor not cleaned when the least amount of food is exposed. Exposed and flooring under equipment and in walk in cooler /walk in freezer in need of cleaning.
36-18-4    Basic - Floor tiles cracked, broken or in disrepair. Upstairs and back room. And at top of stair floor soft. **Repeat Violation**
36-17-4    Basic - Floor tiles missing. Upstairs and back dry stock room under water heater and ice machine. And inside mop sink in back room
23-19-4    Basic - Food debris/dust/grease/soil residue on exterior of reach-in cooler/refrigerator. On cooks line.
14-38-4    Basic - Food storage container/container lid cracked or broken.
23-07-4    Basic - Gaskets with slimy/mold-like build-up. Reach in units on cooks line
22-19-4    Basic - Interior of microwave soiled with encrusted food debris. Both microwave ovens on cooks line.
22-08-4    Basic - Interior of oven has heavy accumulation of black substance/grease/food debris. Hot holding drawers, soiled interior
05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. Walk in cooler, outside thermometer broken, could not find one inside unit.
35B-05-4    Basic - Outer openings not protected during operation and vermin and/or environmental cross contamination present. Upstairs door not fully sealed when closed due to broken and missing plexiglass, gaps in wood door.
14-67-4    Basic - Reach-in cooler gasket torn/in disrepair. 4 door reach in cooler on cooks line
14-33-4    Basic - Reach-in cooler shelves with rust that has pitted the surface.
35B-09-4    Basic - Screen in door torn/in poor repair - vermin present.
23-12-4    Basic - Walk-in cooler and/or walk-in freezer shelves soiled with encrusted food debris.
36-26-4    Basic - Wall soiled with accumulated black debris in dishwashing area. Under the counter, wall and floor.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. 3 plus live flies in kitchen, 1 live fly in dining room area.
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license.
03F-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Butter blend at toaster pulled at 7am not time marked, corrected by cook.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Hard, crumbling, dry, dull rodent droppings on top of water heater and floor of back stock room flooring. 15 or more droppings observed.
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. On arrival as inspector observed pieces of fresh mushrooms and other food debris in hand wash sink in kitchen.
