A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. One dead roach in n hood system crevice on cookline.
14-71-4    Basic - Duct tape used to hang multiple signs throughout kitchen.
12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Drink on prep table by handwash sink. Also two drinks at front counter next to single service items.
36-18-4    Basic - Floor tiles cracked, broken or in disrepair. Floor tiles on cookline. By wall at end of cook line and under steam table
23-20-4    Basic - Food debris/dust/grease/soil residue on exterior of oven. Top of oven at end of cookline has debris buildup.
25-05-4    Basic - Single-service articles improperly stored. Case of styrofoam boxes on floor of dry storage room
08B-14-4    Basic - Stored food not covered in walk-in freezer. Fish in walk-in freezer not covered.
23-12-4    Basic - Walk-in cooler shelves soiled with encrusted food debris. Shelves and speed rack have buildup.
14-17-4    Basic - Walk-in cooler shelves with rust that has pitted the surface. Shelves in walk-in cooler are rusted.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Wet towel on cart by dish area.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. At least 6 flies in kitchen area by back door.
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license. License expired 6/1/2018
03F-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food identified in the written procedure as a food held using time as a public health control has no time marking. Shell eggs, cut tomatoes, lettuce. All items on make table with no time mark, employee stated items stocked within last half hour.
08A-21-4    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature in walk-in cooler. Hamburger stored over ham and fish.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Two live roaches at wall corner crevice by dish machine. Two live roaches in crevice of clean equipment shelves next to walk-in cooler. One live roach crawled on cutting boards on clean equipment shelves. Twenty one live roaches on pvc water line under steam table on cookline.
41-02-4    High Priority - Toxic substance/chemical stored by or with food. Spray can of clean duster on prep table by handwash sink.
29-34-4    High Priority - Vacuum breaker missing at hose bibb. Hose bib on cookline with hose attached does not have a vacuum breaker.
