Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Male employee.
27-10-4    Basic - No hot running water at mop sink. **Corrective Action Taken**
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Hand wash sink next to walk in cooler. **Repeat Violation**
12A-16-4    High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands.
12A-10-4    High Priority - Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee touched her neck then handled pizza cutter.
12A-19-4    High Priority - Employee washed hands with cold water.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed about 12, in dry storage and ware washing areas. Flies were flying around and/or landing on boxes of merchandise. Operator stated delivery truck came at about 10:30 and back door was left open during that time. Operator stated she called the pest control company for service. **Admin Complaint**
31A-03-4    Intermediate - Handwash sink not accessible for employee use due to items stored in the sink. Wet wiping cloth and food debris.
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Employee washed pizza cutter at the hand wash sink.
27-16-4    Intermediate - Hot water not provided/shut off at employee handwash sink. Operator stated hot water went out last night and a service call was made for repairs. Technician did not arrive at time of inspection. **Corrective Action Taken**
53A-05-6    Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. 41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Glass cleaner, degreaser, sanitizer.
