Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. Observed 1 dead roach on wall entrance to kitchen. Observed 6 dead roachs in beer storage room. **Warning**

03D-01-4    High Priority - Cooked potentially hazardous (time/temperature control for safety) food not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within 2 hours. Observed chicken not properly cooled to 70 within 2 hours. Operator uncovered chicken and moved to freezer for rapid cooling. **Corrective Action Taken** **Warning**
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Observed dented top seal on instant mashed potato can. **Warning**
12A-04-5    High Priority - Employee handled soiled dishes or utensils and then handled clean dishes or utensils without washing hands. Observed no hand wash by dishwasher between dirty and clean handling of dishes. **Warning**
12A-18-4    High Priority - Employee wiped hands on clothes/apron/soiled towel after washing. Observed employee wipe hands on soiled towel then continue with food preparation. **Warning**
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed small flying insects near the drains throughout the kitchen area. **Warning**
07-08-4    High Priority - Potentially hazardous (time/temperature control for safety) food re-served to customers. Observed butter packets being left on tables and reserved to customers. **Warning**
08A-21-4    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature in walk-in cooler. Observed raw beef located over fish, and raw chicken located over fish. Emailed operator food storage chart, and operator moved product to proper storage parameters. **Corrected On-Site** **Warning**
03A-04-4    High Priority - Shell eggs held at room temperature with an ambient air temperature greater than 45 degrees Fahrenheit. Observed shelled eggs at 71°F next to flattop. Operator moved eggs to reach in cooler to bring back into proper temperature. **Corrective Action Taken** **Warning**
01B-02-4    High Priority - Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. Observed butter on tables at 75° issued stop sale. **Warning**
41-02-4    High Priority - Toxic substance/chemical stored by or with food. Observed lubricant stored on shelf next to cracker meal. Observed quaternary stored next to pancake mix in old walk in cooler. Observed motor oil stored next to food and over prep table next to ice machine. **Warning**
22-21-4    Intermediate - Accumulation of black/green mold-like substance inside the ice bin. Observed mold like substance on interior of ice machine on shoot area. **Repeat Violation** **Warning**
22-24-4    Intermediate - Accumulation of black/green mold-like substance on/around soda dispensing nozzles. Observed soda dispenser with mold like substance on nozzle area. **Warning**

22-63-4    Intermediate - Accumulation of mold-like substance on juice/drink dispensing nozzles/equipment. Observed mold like substance on juice dispenser nozzle area. **Warning**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Observed hand wash sink near ice machine used for purposes other than hand wash as evidenced by ice in the sink. **Warning**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. No paper towels located at hand wash sink in back hallway. **Repeat Violation** **Warning**
53B-02-5    Intermediate - No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed employees hired over 60 days with no proof of food service certification. This violation must be corrected by November 26 2018. **Warning**
