Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

12B-04-4    Basic - Designated employee eating/drinking/smoking area located in a food preparation or other restricted area causing possible cross contamination. Observed employee drinking coffee behind front counter.
12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee coffee cup on prep table next to food preparation area.
40-06-4    Basic - Employee personal items stored in or above a food preparation area. Observed cell phone on top of counter above food storage.
36-47-5    Basic - Hood soiled with accumulated grease, dust or food debris. .
14-69-4    Basic - Ice buildup in reach-in freezer.
22-08-4    Basic - Interior of oven has heavy accumulation of black substance/grease/food debris.
33-33-4    Basic - Large amount of unused equipment/supplies present. Observed unused equipment in back storage room.
14-31-4    Basic - Nonfood-grade bags used in direct contact with food. Observed tamales stored in "to go" bags.
29-08-4    Basic - Plumbing system in disrepair. Observed pipe at mop sink broken, water flowing onto floor.
14-67-4    Basic - Reach-in freezer gasket torn/in disrepair.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed salt in container with no labeling. **Corrected On-Site**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed fries (62°F - Cold Holding at room temperature); raw bacon (62°F - Cold Holding). Manager moved items to freezer for rapid chill. **Corrective Action Taken**
08A-05-4    High Priority - Raw animal food stored over ready-to-eat food. Observed raw bacon stored over ham croquettes. Manager rearranged for proper storage. **Corrected On-Site**
12A-09-4    High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee touch garbage can to move then touched to go containers to serve food. Coached employee on proper hand washing procedures. **Corrective Action Taken**
29-42-4    High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.
22-28-4    Intermediate - Interior of reach-in freezer soiled with accumulation of food residue. In back.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed degreaser with no label. Employee labeled. **Corrected On-Site**
