Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

14-01-4    Basic - Bowl or other container with no handle used to dispense food. Observed a plastic container used to dispense sauce.
24-24-4    Basic - Clean equipment and utensils stored in a room that is not fully enclosed (open to outdoors/screened). Observed some clean plastic containers used to prep and store food in a room that is not fully enclosed. The room has screen in some parts of the walls. **Repeat Violation**
16-55-4    Basic - Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Observed warewashing machine out of service. According owner 1 pipe broke down some days ago, so it is not in use.
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Observed owner and employee with no hair restrain engaged in food preparation. **Repeat Violation**
08B-40-4    Basic - Food being prepared in a room that has screening in place of all or part of a wall. Observed employee peeling raw shrimps above a table in a back room that is not fully enclosed and has screen on some walls.
23-21-4    Basic - Food debris/dust/grease/soil residue on exterior of microwave. Observed grease on the exterior of the microwave.
14-38-4    Basic - Food storage container/container lid cracked or broken. Observed cracked containers used to store food.
10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed utensil in standing water at 79°f.
14-31-4    Basic - Nonfood-grade bags used in direct contact with food. Observed raw chicken stored directly on Walmart shopping market bag in a reach in cooler. **Repeat Violation**
08A-26-4    Basic - Raw animal food stored above unwashed produce. Observed raw chicken above unwashed produce inside walking cooler.
14-33-4    Basic - Reach-in cooler shelves with rust that has pitted the surface.
36-02-5    Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. All throughout the kitchen. **Repeat Violation**
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. ( all the shelves in the walking cooler).
12A-20-4    High Priority - Employee washed hands with no soap in other than approved handwash sink. Employee was coached about handwashing procedure.
22-45-4    High Priority - Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. Observed employee washing a food container inside a single sink with no soap and without sanitizer. The sink is located in the back screened room that it is not fully enclosed.
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed bean sprouts at 74°f inside a water container above a kitchen counter for approximately 2 1/2 hrs according owner because the restaurant opened late today. Owner transferred the bean sprouts to the walking cooler. **Corrective Action Taken** **Repeat Violation**
08A-10-4    High Priority - Raw animal food stored over ready-to-eat food in reach-in cooler. Observed raw chicken above cooked egg rolls inside reach in cooler in the kitchen.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 15 + dry droppings in a room used to keep maintenance supplies and cardboard boxes for deliveries.
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed hose connected and no backflow preventer.
22-28-4    Intermediate - Interior of reach-in cooler soiled with accumulation of food residue. Observed accumulation of old food debris inside reach in cooler located at the left side of the kitchen entrance. **Repeat Violation**
14-77-6    Intermediate - Reach-in cooler not maintained in good repair. Do not store potentially hazardous (time/temperature control for safety) food in this unit until the unit is repaired. Observed empty reach in cooler out of service. According owner the reach in cooler broke last week.
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Observed cooked sweet and sour chicken from Sunday 3/24 according owner without date inside reach in cooler. **Repeat Violation**
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed a plastic spray bottle with degreaser with no label.
16-28-4    Intermediate - Two-compartment sink used for warewashing. Observed warewashing machine is broken and only 2 compartment sink in the establishment.
