Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated grease, dust, or mold-like substance. **Repeat Violation**

12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee lunch meal in a bowl on top of the reach in cooler.
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Observed employees not wearing hair restraint and engage in food preparation.
14-11-4    Basic - Equipment in poor repair. Observed a cracked pitcher used for rice has black taping around it sitting on a shelf.
36-22-4    Basic - Floor area(s) covered with standing water. At the front counter.
36-18-4    Basic - Floor tiles cracked, broken or in disrepair. At the front counter.

23-22-4    Basic - Food debris/dust/soil residue on dry storage shelves.
08B-47-4    Basic - Food not stored at least 6 inches off of the floor. Observed cases of plastic soda bottles on the floor of the back entrance.
36-14-4    Basic - Grease accumulated on kitchen floor and/or under cooking equipment. Throughout the kitchen area. **Repeat Violation**
10-01-4    Basic - In-use utensil in nonpotentially hazardous (non-time/temperature control for safety) food not stored with handle above top of food within a closed container. Observed a bowl in the flour container in the kitchen.
38-01-4    Basic - Light shield damaged/in disrepair. Observed light shield cracked in the kitchen over the reach in cooler.
14-31-4    Basic - Nonfood-grade bags used in direct contact with food. Observed spinach and squash in a non food grade bag in the reach in freezer.
06-04-4    Basic - Potentially hazardous (time/temperature control for safety) food thawed at room temperature. Observed 2 packs of salmon in pan sitting on chest freezer at front counter.
23-14-4    Basic - Shelf under preparation table soiled with food debris.
23-09-4    Basic - Soiled reach-in cooler/freezer gaskets.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Throughout the kitchen area. **Repeat Violation**
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. **Repeat Violation**
22-18-4    Intermediate - Soil residue in food storage containers. Observed flour, rice, soy sauce, and sesame seed containers are soiled.
