Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drink without a lid stored on prep table next to cheese shredder. Item moved during inspection. **Corrected On-Site** **Repeat Violation**
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed accumulation of food debris in corners and along wall under and behind equipment in prep area. Also, buildup of debris on floor of outside storage shed**Repeat Violation** **Repeat Violation**
08B-38-4    Basic - Food stored on floor. Case of oil stored on floor in kitchen area. Case of oil store on floor in dry storage shed. Bags of rice stored on floor next to ice machine. **Repeat Violation**
33-19-4    Basic - Garbage on the ground and/or pad around dumpster. Observed trash under dumpster in parking lot. Debris include cardboard cheese box and black trash bag full of trash. Observed debris/trash in parking lot. Debris includes cardboard box, discarded food container caps, two cloth towels, discarded pvc pipe, and semi-dissolved white paper product.
33-16-4    Basic - Open dumpster lid.
06-01-4    Basic - Potentially hazardous (time/temperature control for safety) food thawed in an improper manner. Raw chicken, raw shrimp. Both items stored in prep sink in prep area. Shrimp in pan with standing water. Chicken in bag at room temperature.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Multiple wet, soiled cloths stored on prep tables in back prep area.
22-42-4    High Priority - Chlorine sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. At three compartment sink. Test strip instantly bleach back white before reading 200+ on operators strip. Discussed sanitizer level with employee working in area.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed 2 live flies in kitchen area. Observed 4-5 live flies in outside storage shed. Food product and single service items stored in shelf along with chest freezers. Door to shed left open. **Repeat Violation**
03B-01-5    High Priority - Potentially hazardous (time/temperature control for safety) food hot held at less than 135 degrees Fahrenheit or above. Rice 113°. Queso 120° items in pans placed on top of lids of product inside of steam well. Employee discarded product during inspection. Discussed temperature requirements with employee
41-10-4    High Priority - Toxic substance/chemical improperly stored. Two bottles of fabuloso cleaner stored on prep table next to cheese shredded. Aerosol pest spray stored on prep table in back area next to slicer. **Repeat Violation**
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Beans, rice in stand up reach in cooler in back area. Item made Saturday not properly datemarked. Per employee item was not labeled due to the absence of Saturday day dots.
