Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Approximately 25 lives flies at front pasta station cooler, landing on cooler, take out containers above cooler, surrounding walls, empty pasta well and squeeze bottle of water. Approximately 3 dozen live flies landing on sneeze guard at front counter hot side. Approximately 10 live flies hovering front flip top reach in cooler. Approximately 10 live flies landing on soda machine levers and fruit juice dispenser containers. Approximately 3 live flies hovering cold front line. 1 dead fly on HWS in men's restroom, 2 dead flies on HWS in women's restroom. Approximately 1 dozen on walls in dining room, approximately 10 on lights hanging over tables in dining room, Globe style light covers over tables in dining room filled with approximately 80 dead flies in light globes. **Warning**

At front cold line, overnight oats (54°F - Cold Holding); yogurt (55°F - Cold Holding); rice pudding (54°F - Cold Holding). Operator states less than 2 hours. Operator states he is using TPHC. Reviewed on previous inspection, no time mark or previously provided written form available. In flip top reach in cooler, roasted beets (46°F - Cold Holding); stuffed grape leaves (45°F - Cold Holding).
