Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-37-5    Basic - Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas.
36-36-4    Basic - Ceiling tile missing, kitchen area.
36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair, kitchen area. **Repeat Violation**
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance, kitchen area
24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed cleaned pots stored not inverted.
16-55-4    Basic - Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Operator will use three compartment in the mean time dish machine gets fix.
16-13-5    Basic - Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized.
36-71-4    Basic - Floor drains/drain covers heavily soiled.
36-01-4    Basic - Floor not cleaned when the least amount of food is exposed.
36-73-4    Basic - Floor soiled/has accumulation of debris, in walk-in freezer and cook line.
23-21-4    Basic - Food debris/dust/grease/soil residue on exterior of microwave.
23-19-4    Basic - Food debris/dust/grease/soil residue on exterior of reach-in cooler/refrigerator, located in kitchen area.
08B-38-4    Basic - Food stored on floor. Observed case of cheese bread stored on floor in walk-in cooler. **Repeat Violation**
36-14-4    Basic - Grease accumulated on kitchen floor and/or under cooking equipment.
33-20-4    Basic - Grease on the ground and/or pad around grease receptacle.
36-47-5    Basic - Hood soiled with accumulated grease, dust or food debris.
14-69-4    Basic - Ice buildup in reach-in freezer. **Repeat Violation**
10-17-4    Basic - In-use knife/knives stored in cracks between pieces of equipment. Observed utensils stored between wall and three compartment sink.
10-20-4    Basic - In-use tongs stored on equipment door handle between uses.
22-19-4    Basic - Interior of microwave soiled with encrusted food debris.
14-36-5    Basic - Interior of refrigerator or freezer in disrepair/has exposed insulation. Observed reach in freezer in disrepair, located next to cook line.
33-11-4    Basic - Missing drain plug at dumpster.
16-46-4    Basic - Old labels stuck to food containers after cleaning.
33-16-4    Basic - Open dumpster lid.
14-67-4    Basic - Reach-in cooler gasket torn/in disrepair, located in kitchen area.
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Observed to go containers not inverted in kitchen area.
23-09-4    Basic - Soiled reach-in cooler gaskets, located in kitchen area.
14-41-4    Basic - Walk-in cooler and walk-in freezer gasket torn/in disrepair.
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed walk-in cooler and walk-in freezer shelves rusted.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed wall soiled in kitchen area.
29-11-4    Basic - Water leaking from pipe and/or faucet/handle, from three compartment sink.
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed approximately 10 live roaches on the floor in the cook line area.
22-28-4    Intermediate - Interior of reach-in cooler soiled with accumulation of food residue, kitchen area.
22-18-4    Intermediate - Soil residue in food storage containers. Observed containers of beans, sugar, bread crumbs, soiled. **Repeat Violation**
