Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Observed gap at threshold of back door, located next to walk in freezer.
36-41-4    Basic - Fan cover in walk-in cooler/freezer has accumulation of dust or debris. Observed accumulation of dust on walk in coolers, located throughout kitchen.
36-12-4    Basic - Floors not constructed to be easily cleanable. Observed grout and tiles missing or in disrepair throughout kitchen. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee. **Repeat Violation**
08B-38-4    Basic - Food stored on floor. Observed oysters stored on floor, located in walk in cooler across from prep area. Operator moved oysters. **Corrected On-Site**
36-47-5    Basic - Hood soiled with accumulated grease, dust or food debris. Observed accumulation of grease on hood, located above cook line.
10-12-5    Basic - In-use ice scoop stored on soiled surface between uses. Observed ice scoop sitting on cardboard, located next to walk in freezer.
22-19-4    Basic - Interior of microwave soiled with encrusted food debris. Observed interior of microwave with encrusted food debris, located next to hand washing sink.
38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Observed missing light cover, located over prep area across from walk in cooler.
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Observed no employees must wash hands at multiple hand washing sinks, located throughout kitchen. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee. **Repeat Violation**
16-46-4    Basic - Old labels stuck to food containers after cleaning. Observed old labels on clean bins, located across from cook line.
08B-12-5    Basic - Stored food not covered. Observed cole slaw and potatoes with no cover stored in walk in cooler, located by cook line and across from prep area.
36-50-4    Basic - Unclean building components, attachments or fixtures. Observed food build up on water sprinklers and outlets, located throughout kitchen.
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed rust on shelves on walk in cooler and reach in cooler, located throughout kitchen.
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Observed faucet leaking at hand washing sink, located next to cook line.
14-15-4    High Priority - Nonfood-grade containers used for food storage - direct contact with food. Observed nonfood grade containers holding bread crumbs, located in walk in cooler across from prep area.
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed cole slaw above 41 °F in reach in cooler, located across from cook line. Operator said it has been in reach in cooler for 3 days. Operator discarded item. cole slaw (44°F - Cold Holding)

22-24-5    Intermediate - Accumulation of black/green mold-like substance on/around soda dispensing nozzles. Observed green mold like substance around soda nozzles, located at wait station.
01C-05-4    Intermediate - Clam/mussel/oyster tags not maintained in chronological order according to the last date they were served in the establishment. Observed oysters tags not in chronological order from the last date served.

02C-03-5    Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed milk not date marked after opening.
22-22-4    Intermediate - Encrusted material on can opener blade. Observed can opener blade with encrusted material, located across from walk in cooler.
31B-03-4    Intermediate - No soap provided at handwash sink. Observed no soap at hand washing sink, located across from cook line. Operator replaced soap. **Corrected On-Site**
02A-01-4    Intermediate - Raw or undercooked oysters offered and establishment has no consumer advisory sign provided on wall, menu, placard, table tent or by any other written means. Raw animal foods must be fully cooked prior to service. Observed no consumer advisory for oysters. Provided Operator with sign which he posted. **Corrected On-Site**

41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed unlabeled spray bottle (sanitizer), located in kitchen.
