Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

23-06-4    Basic - Build-up of food debris, dust or dirt on nonfood-contact surface. Observed underneath cooking equipment on main line, under dishwasher, under front counter bar area and wait station.

51-11-4    Basic - Carbon dioxide/helium tanks not adequately secured. Observed near back door.
36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed next to back door.
24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed soup bowls in main kitchen not inverted.
32-12-5    Basic - Covered waste receptacle not provided in women's bathroom.
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Observed in main kitchen area near the walk in cooler and near the back door. **Repeat Violation**
33-14-4    Basic - Dumpster overflowing garbage. Observed overflowing.
12B-02-4    Basic - Employee eating in a food preparation or other restricted area. Observed coffe drinking in main kitchen. Observed operator disposed at time of inspection. **Corrected On-Site**
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Observed gap at back door in kitchen. **Repeat Violation**
14-23-4    Basic - Fixed equipment exposed to spillage not sealed to adjoining equipment/walls. Observed at hand washing sink behind cook line. **Repeat Violation**
36-01-4    Basic - Floor not cleaned when the least amount of food is exposed. Observed behind the cook line in the main kitchen.
14-38-4    Basic - Food storage container/container lid cracked or broken. Observed on bin containing dry black beans
08B-38-4    Basic - Food stored on floor. Observed plastic container of oil on the floor in the kitchen and bread on the floor at the front wait station. **Repeat Violation**
23-07-4    Basic - Gaskets with slimy/mold-like build-up. Observed on reach in cooler in main kitchen.
14-69-4    Basic - Ice buildup in reach-in freezer. Observed in ice cream freezer at wait station and walk in freezer.
10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed in rice and brown rice bins next to stack ovens.
33-11-4    Basic - Missing drain plug at dumpster.
36-34-5    Basic - Observed Ceiling/ceiling tiles/vents and support cables hanging from ceiling are soiled with accumulated food debris, grease, dust, or mold-like substance.
16-46-4    Basic - Old labels stuck to food containers after cleaning. Observed on plastic containers in main kitchen dish area.
23-05-4    Basic - Soil residue build-up on nonfood-contact surface. Observed soiled plastic containers with Rice, brown rice and seasonings outside of walk in cooler next to the oven.
29-49-6    Basic - Standing water in bottom of reach-in-cooler. Observed in main kitchen cooler.
42-03-5    Basic - Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. Observed floor drain snake stored above food on storage shelves next to back door.
08B-12-5    Basic - Stored food not covered. Observed sugar at the coffee station on front bar and dessert in the reach in cooler on wait station.
08B-54-4    Basic - Uncovered food stored near sink exposed to splash. Observed bacon next to hand sink in main kitchen.
08B-17-4    Basic - Unwashed fruits/vegetables stored with ready-to-eat food. Observed unwashed turnips stored on top of milk containers in walk in cooler.
23-12-4    Basic - Walk-in cooler and/or walk-in freezer shelves soiled with encrusted food debris. Observed in walk in cooler and reach in cooler under the main cook line.
14-41-4    Basic - Walk-in cooler gasket torn/in disrepair. Observed on door to cooler in main kitchen.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed near the hand washing sink in the kitchen and dishwashing area near the main line. **Repeat Violation**
29-03-4    Basic - Water draining onto floor surface. Observed next to stack ovens.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed at front wait station. **Repeat Violation**
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Observed 2 cans of grave marmalade on storage shelves in main kitchen.
08A-02-6    High Priority - Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Observed raw pork over croquettes in the freezer and raw beef stored over sliced cooked turkey and raw eggs over raw pork in walk in cooler. Operator rearranged at time of inspection. **Corrected On-Site**
08A-05-6    High Priority - Raw animal food stored over ready-to-eat food. Observed bacon over croquettes in freezer, and raw beef over croquettes in walk in freezer. Operator rearranged at time of inspection. **Corrected On-Site**
08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit. Observed raw eggs stored over raw pork in the walk in cooler.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit., and chimichuri (49°F cold holding) on the front expo station on the cook line. operator put bags of ice on products at time of inspection. **Corrective Action Taken**
29-42-4    High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed vacuum breakers missing from mop sink area.
22-02-4    Intermediate - Accumulation of food debris/grease on food-contact surface. Observed on storage bins of flour and black beans in main kitchen.
14-74-7    Basic - Intermediate - Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed reach in cooler on cook line not keeping foods to proper temperature.
22-05-4    Intermediate - Cutting board(s) stained/soiled. Observed in main kitchen area.
22-31-4    Intermediate - Non-pitting surface rust on food-contact equipment. Observed on scale in kitchen.
22-01-4    Intermediate - Soda gun soiled. Observed at soda dispensing area at wait station.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed degrease not labeled in kitchen and sanitizer at the wait station.
