Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

23-24-4    Basic - Buildup of food debris/soil residue on equipment door handles. Flip top cooler next to ice machine has soiled handles on topside and underside with old food debris.
36-36-4    Basic - Ceiling tile missing. Ceiling tile missing between walk in cooler and walk in freezer.
33-16-4    Basic - Open dumpster lid. Dumpster lid open. **Repeat Violation**
14-67-4    Basic - Reach-in cooler gasket torn/in disrepair. Flip top reach in cooler next to ice machine has torn gasket.
29-19-4    Basic - Standing water in floor drain/floor drain draining very slowly. Floor drain between three compartment sink and mop sink is not draining. Mop sink is not draining and three compare sink drain is not draining. **Warning**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Walls in warewashing area soiled with debris and residue
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Two small gnat like insects near ice machine.
28-26-4    High Priority - Sewage/wastewater backing up through floor drains. Mop sink, three compartment sink and floor drain are not draining and backing up. The overflow of grey water is filling the warewashing area and flooding over to a drain outside the walk in cooler. Inspector observed Employees walking through water to enter the walk in cooler and exit the rear door. Excess overflow is being pushed into another drain and there is evidence of water being pushed out the rear door. **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Coleslaw 53°F in small flip top cooler next to ice machine. Operator moved slaw into walk in cooler. **Corrective Action Taken**
22-24-5    Intermediate - Accumulation of black/green mold-like substance on/around soda dispensing nozzles. Soda nozzles in customer area and drive window have soiled accumulation on topside of nozzle area
31B-03-4    Intermediate - No soap provided at handwash sink. No soap at hand wash sink next to drive thru window
