Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Observed single service plates and lids not stored inverted or protected. **Corrected On-Site**
33-31-5    Basic - Unnecessary items/unused equipment on the premises. Observed 2 reach in coolers located by the dry storage area as per operator they are not using the coolers. **Repeat Violation**
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license. **Admin Complaint**
02C-03-5    Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed tofu with no date marked as per operator since Sunday in the cooler located opposite stove.

31A-03-4    Intermediate - Handwash sink not accessible for employee use due to items stored in the sink. Observed containers stored in the hand sink located at the bar area.

02B-01-5    Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Observed on to go menu and online menu cold appetizers and cucumber wraps this food is make with raw fish, as per operator they are not doing sushi, at the time of the inspection provided operator consumer advisory sign. **Corrective Action Taken**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Bar area.
53B-01-5    Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. At the time of the inspection establishment is closed to the public as per operator he has 3 employees working with him.
