Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

16-16-4    Basic - Equipment and utensils not rinsed between washing and sanitizing. Observed equipment and utensils being used. **Repeat Violation**
36-71-4    Basic - Floor drains/drain covers heavily soiled. **Repeat Violation**
36-01-4    Basic - Floor not cleaned when the least amount of food is exposed. Observed kitchen, dining room and front counter floor has accumulation of old food debris. **Repeat Violation**
36-14-4    Basic - Grease accumulated on kitchen floor and/or under cooking equipment. **Repeat Violation**
36-64-5    Basic - Objectionable odors in bathroom or other areas of the establishment. **Repeat Violation**

23-14-4    Basic - Shelf under preparation table soiled with food debris. Observed shelves under preparation tables are soiled with old food debris and dust. **Repeat Violation**
23-09-4    Basic - Soiled reach-in cooler gaskets. Observed all gaskets for chest freezers and reach in coolers has soiled build up. **Repeat Violation**
29-49-6    Basic - Standing water in bottom of kitchen reach-in-cooler.
08B-12-5    Basic - Stored food not covered observed shrimp, veggie rolls, dumplings, and steak in the reach in cooler uncovered. Chef covered all foods. **Corrected On-Site**
36-26-4    Basic - Wall soiled with accumulated black debris in dishwashing area. Observed all wall areas has food debris, grease and dust. **Repeat Violation**
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloths on front counter and kitchen preparation tables. **Repeat Violation**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed a live roach crawling on closed, cooked rice container at the front counter, and 2 live roaches crawling on the wall at the front counter. **Admin Complaint**
22-02-4    Intermediate - Accumulation of food debris/grease on food-contact surface. Observed accumulation of food debris on the stove, front counter preparation table, table oven at the front counter, grill and rice cooker. **Repeat Violation**
22-28-4    Intermediate - Interior of reach-in cooler soiled with accumulation of food residue. Observed interior of reach in cooler shelves, and freezer is soiled with accumulation of food debris. **Repeat Violation**
22-30-4    Intermediate - Interior of refrigerator soiled with accumulation of food residue. Observed reach in cooler has accumulation of food debris. **Repeat Violation**
16-35-4    Intermediate - No chlorine chemical test kit provided when using chlorine sanitizer at three-compartment sink/warewashing machine. Observed no chlorine test kit. Operator has quaternary test kit but no quaternary solution. **Repeat Violation**
53B-01-5    Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed Employees Carlos Sanchez, Raphael Blanco, Illana Oginskis has no proof of required state approved employee training certificates. **Admin Complaint**
