Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

23-04-4    Basic - Build-up of mold-like substance on nonfood-contact surface. Observed white and black mold-like substance buildup on the fan guards of the walk-in refrigerator blower unit.
51-11-4    Basic - Carbon dioxide/helium tanks not adequately secured. Observed CO2 tanks are not chained in the beverage area.
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed browning/ blackening of ceiling in the kitchen area.
14-71-4    Basic - Duct tape used to repair nonfood-contact surface. Observed duct tape on the handle of the rice warmer
38-11-4    Basic - Fifty (50) foot-candles of light not provided at a food working surface or where safety is a factor. Observed kitchen dimly lit and lacking adequate lighting to meet 50 foot candles of light in a food preparation area. **Repeat Violation**
08B-19-4    Basic - Food contaminated by unsanitized equipment. Observed cut prepared broccoli returned to the original unclean box. Broccoli rewashed and placed in a clean and sanitized container. **Corrected On-Site** **Repeat Violation**
14-13-4    Basic - Food-contact surface not smooth and easily cleanable. Observed a paint mixer attachment in a drill that the operator indicates he uses to mix food. Operator removed from the kitchen. **Corrected On-Site**
36-24-5    Basic - Hole in or other damage to wall. Observed rusting of walls and rough paint on the door of the walk-in refrigerator.
10-08-5    Basic - Ice scoop handle in contact with ice. Observed the handle of the ice scoop laying on the ice in the soda dispenser ice bin. Discussed properly placing with the handle out of the ice. **Corrected On-Site**
23-05-4    Basic - Soil residue build-up on nonfood-contact surface. Observed heavy blackened grease buildup on the wok range, on the small food cart by the wok, on shelves on preparation tables, on dry storage shelves, and the walk-in refrigerator shelving. **Repeat Violation**
23-09-4    Basic - Soiled reach-in cooler gaskets. Observed wet slimy substance in the door gaskets of the refrigerated make table on the cook line
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Observed fish, chicken in standing water to thaw. Observed imitation crab meat packages thawing on a preparation table
08B-17-4    Basic - Unwashed fruits/vegetables stored with ready-to-eat food. Observed unwashed mushrooms over prepared celery, onions, and cooked noodles. Operator rearranged the containers on the shelves to correct. **Corrected On-Site**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed brown substance buildup on the walls of the walk-in refrigerator.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Wet wiping cloths left on the counters and not returned to sanitizer solution between uses.
36-10-4    Basic - Worn, torn and/or soiled floors/carpeting. Observed food, grease, and debris buildup on the floor under equipment in the kitchen, under shelves and pallets in the walk-in freezer, and under shelves and pallets in the walk-in refrigerator. **Repeat Violation**
03D-01-5    High Priority - Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within 2 hours. Observed a bus tub full of noodles at 47°F in the walk-in refrigerator. The noodles are 6 inches deep in the container. The operator indicates and labeled the noodles as cooked yesterday, 4/26. Won ton filling in the same refrigerator measures 42°F. Stop sale issued for the improperly cooled noodles.
08A-04-5    High Priority - Raw animal food stored over or with unwashed produce. Observed raw beef over uncovered and unwashed peppers. Observed raw chicken over unwashed celery. Operator rearranged the food on the shelves off the walk-in refrigerator to properly locate raw items. **Corrected On-Site** **Repeat Violation**
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed cooked noodles from the previous day that measure 47°F today., more than 12 hours later.
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed a water spigot at the mop sink that lacks a backflow preventer **Repeat Violation**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed that paper towels or other approved drying device are absent from the hand washing sink by the walk-in refrigerator. Paper towels provided. **Corrected On-Site** **Repeat Violation**
22-16-4    Basic - Intermediate - Reach-in cooler shelves soiled with food debris. Observed grease and debris buildup on the wire shelving in the 2 door reach in refrigerator **Repeat Violation**
