A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas.
Acoustic style tiles present in kitchen area are not painted/sealed. **Repeat Violation**
Basic - Dead roaches on premises.
Observed approximately 37 dead roaches in establishment: 11 in bar area under bar top on floor, 2 in server station in dining area, 7 on floor near pass through window in kitchen, 6 under bag in box rack near walk in cooler in kitchen, 6 under and around ice machine and 5 in dish area in kitchen. Manager began removing dead roaches. **Corrective Action Taken**
Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items.
Employee soiled aprons and jackets stored on dry food storage shelves in back area.
Basic - Floors, walls, and/or ceilings soiled/has accumulation of debris.

Basic - Wet wiping cloth not stored in sanitizing solution between uses.

High Priority - Fish not held frozen before, during and after being packaged using a reduced oxygen packaging method.
Fish in cooler drawers on cook line. Fish removed from packaging. **Corrected On-Site**
High Priority - Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged.
Raw beef stored over cooked chili peppers in walk in freezer. Items moved and stored correctly. **Corrected On-Site**
High Priority - Roach activity present as evidenced by live roaches found.
Observed 14 live roaches in establishment: 4 on top of ice machine in kitchen area, 3 on wall behind ice machine, 3 inside warming oven where rice is kept warm, 2 near floor drain by dish area, and 2 on wall in dish area. Manager killed 3 roaches and discarded. **Corrective Action Taken** **Admin Complaint**
High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit.
Salsa in server area 69°F. Salsa placed out approximately 15 minutes prior per employee. Salsa placed back into walk in cooler to cool. **Corrective Action Taken** **Repeat Violation** **Admin Complaint**
Intermediate - Employee with artificial nails/nail polish working with exposed food without wearing intact gloves.
Multiple servers working during inspection. Servers put gloves on. **Corrected On-Site**
Intermediate - Handwash sink not accessible for employee use due to items stored in the sink.
