A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed multiple ceiling tiles with water damaged above kitchen area and also dry storage room.
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed accumulated dust on fans, vents and ceilings.
21-05-5    Basic - Cloth used as a food-contact surface. Observed to cover food inside reach in cooler. Manager removed. **Corrected On-Site**
08B-49-4    Basic - Employee personal food not properly identified and segregated from food to be served to the public.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed glasses, mask and some personal items stored above dishwasher table.
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside.
36-08-4    Basic - Floor and wall junctures not coved in food preparation/storage/warewashing areas or bathroom. Observed cove molding missing at some places of sushi bar.
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed behind equipments at kitchen and also behind horizontal freezer at storage area.

14-69-4    Basic - Ice buildup in reach-in freezer. Observed at horizontal freezer at dry storage.
10-17-4    Basic - In-use knife/knives stored in cracks between pieces of equipment. Observed 2 knives stored between equipment at kitchen cook line. Manager relocated. **Corrected On-Site**
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Observed At kitchen oven handle. Manager removed. **Corrected On-Site**

14-67-4    Basic - Reach-in cooler gasket torn/in disrepair. Observed at reach in freezer next to employees bathrooms.
25-35-4    Basic - Reuse of single-use articles. Observed mayo container been reused at sushi area.
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Observed to go container facing up at kitchen. Operator inverted. **Corrective Action Taken**
23-09-4    Basic - Soiled reach-in cooler gaskets. Observed at sushi bar area.
08B-12-5    Basic - Stored food not covered. Observed breaded shrimp not covered inside reach in cooler.
33-31-5    Basic - Unnecessary items/unused equipment on the premises. Observed togo containers, plastic pallets, maintenance material stored inside employees bathroom making unable to access facilities.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed behind reach in freezer at sushi area.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed at preparation table at kitchen area.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed container with sugar not labeled at dry storage room.
14-31-5    High Priority - Nonfood-grade bags used in direct contact with food. Observed fish stored inside nonfood grade bags inside reach in cooler.
08A-04-5    High Priority - Raw animal food stored over or with unwashed produce. Observed raw fish over unwashed produce. Operator relocated. **Corrected On-Site**
03A-04-4    High Priority - Shell eggs held at room temperature with an ambient air temperature greater than 45 degrees Fahrenheit. Observed at kitchen preparation table at kitchen area. As per operator less than 1 hrs. Manager relocated inside reach in cooler. **Corrected On-Site**

31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed at sushi station paper towel dispenser. Manager provided paper towel for station. **Corrected On-Site**
