A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

Operator discarded and washed lid. **Time Extended**
14-36-5    Basic - - From initial inspection : Basic - Interior of refrigerator or freezer in disrepair/has exposed insulation. Observed three chest freezers with exposed insulation next to walk in cooler **Warning** - From follow-up inspection 2021-05-14: Observed same. **Time Extended**
08A-05-6    High Priority - - From initial inspection : High Priority - Raw animal food stored over ready-to-eat food. 1)Observed raw chicken stored above cut ginger in walk in cooler. 2) Observed raw shell eggs stored above cooked chicken in walk in cooler **Warning** - From follow-up inspection 2021-05-14: 1.) Observed container of raw beef and raw chicken stored over container of raw squid and raw pork. Operator went to store properly. Corrective action taken. 2.) Observed containers of raw pork next to container of unwashed mangoes in walk in cooler. Operator removed and stored properly. corrected on site. **Time Extended**

Observed 1 live roach on prep table in front of walk in cooler 1 live roach in kitchen under rice cooker 5 live in clean and sanitized food containers on storage shelf in front restroom next cookline. 1 live roach on wall next to clean food containers on cookline Approximately 20 live roaches on storage rack where clean sanitized food containers are stored next to rice cooker Approximately 10 live roaches under cook line 5 live roaches in gasket of M3 flip top cooler in front of cookline 2 live roaches in electrical switch next to hot water heater by back door 1 live roach in rice bin behind walk in cooler 4 live live on shelf where box syrups are stored in wait station **Warning** - From follow-up inspection 2021-05-14: -Observed 1 live roach over M3 Turbo Air Refrigerator- Operator attempted to kill - Observed 1 live roach on shelving unit on cook line -Observed 1 live roach on ceiling above ice machine- Operator killed and discarded - Observed 1 live roach under wooden stool where fan is stored. Operator killed and discarded. -Observed 1 live roach under prep table crawling on wall. Operator killed and discarded. -Observed 1 live roach under triple sink. Operator killed. **Time Extended**
