A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.

-Observed approximately 4 dead roaches under triple sink.
-Observed 1 dead in trash bin in kitchen next to prep table
-Observed approximately 4 dead in hallway leading to bathrooms.
-Observed approximately 20 dead roaches under sushi bar counters.
-Observed approximately 8 dead roaches on lid of dry Star Anise. Operator discarded and washed lid.
**Time Extended** - From follow-up inspection 2021-05-14: -Observed 2 dead roaches under rack on dry storage area on floor. Operator removed and discarded.
- Observed 1 dead roach under pot and pan rack on floor next to M3 Turbo Air refrigerator in kitchen. Operator removed and
discarded.
-Observed 1 dead roach on pipe under flip top on cook line. Operator removed and discarded.
-Observed approximately 3 dead roaches on metal landing above wok station. Operator removed and discarded, sanitized area.
- Observed approximately 4 dead roaches under rack on floor where containers of plates are stored. Operator removed and discarded.
