A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

Observed approximately 6+ live roaches crawling under a table in the kitchen area located next to walk in cooler used as storage, 1 live roach crawling on a pipe next to a prep table by kitchen northern back door, 1 live roach crawling on a cutting board with broccoli, 1 live roach on a tablecloth that sitting on the steam table, 4 live roaches crawling on a shelf of a steam table, 4 live roach on the kitchen floor in front of the walk in cooler, 9+ live roaches crawling on the kitchen floor of the back hallway area, 15 live roaches on a shelf in the dry storage area room, 2 live roaches inside the prep sink located by northern kitchen door, 1 live roach crawling inside the mop sink located in the kitchen across from chest freezer, 4 live roaches crawling on a shelf that has cup and lids, and napkins not protected, 1 live roach crawling on the wall next to the kitchen bathroom. Observed 1 live roach crawling out of the box of cling wrap. 5+ live roaches crawling inside the three compartment sink, 10+ live roaches crawling into a hole by the entrance of the kitchen door. **Warning** - From follow-up inspection 2021-05-26: At time of call back inspection observed approximately 4+ live roaches on the floor into a hole in the wall by the kitchen door entrance, 10+ live roaches crawling on a wall into a hole in the kitchen area, next to the prep sink, 2+ live roaches crawling under the three compartment sink, 10+ live roaches crawling on the kitchen floor of the back hallway area, 40+ live roaches under bins that has and stores flour, sugar, and rice in the dry storage area room. **Time Extended** - From follow-up inspection 2021-05-27: Observed 1 live roach crawling on the wall located in front of two compartment sink, observed 2 live roaches crawling on kitchen floor, 2 live roaches crawling on the kitchen wall behind the chest reach in freezer located in the main kitchen next to back door, 2 live roaches crawling on the dry storage ceiling, one live roach crawling on the pipe above preparation table, observed 8+ live roaches inside a hole on the ceiling above two compartment sink, 3 live roaches crawling on dishwasher area floor. **Time Extended** - From follow-up inspection 2021-06-01: Observed 2 lives roaches on the wall by the dish machine, 5+ on prep table shelves and 7 on the floor that is closest to the dish machine, 6+ by the rice cooker wall, 2 on the cook line reach in cooler, 2 by the microwave, 3 on the floor by the 2-comp sink, 2 by the dry storage shelf, 2 on the floor by the walk in cooler, 4 by the mop sink and 1 by back door.. **Time Extended**
