Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

13-07-4    Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed several employees working with watches on the cook line.
08B-43-4    Basic - Food stored in a room/shed that is not fully enclosed. Observed boxes of hamburgers buns stored outside the back door of the restaurant underneath tent.
14-70-4    Basic - Ice buildup in walk-in freezer. Observed terrible ice build up on the storage shelves inside walk in freezer.
51-13-4    Basic - No Heimlich maneuver/choking sign posted. Observed no choking poster provided in the restaurant.
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Observed no hand wash sign provide for handwash sink located by the cook line. **Repeat Violation**
16-46-4    Basic - Old labels stuck to food containers after cleaning. Observed old labels on plastic pans stacked wet on the dry shelve in the dishmachine area.
25-05-4    Basic - Single-service articles improperly stored. Observed plastic plates stored next to bag for employees, near locker room.
12A-07-5    High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee working at work station change task touching raw meat then ready to eat bun without the washing hands located on the cook line when changing gloves. **Corrected On-Site**
35A-09-4    High Priority - Presence of insects, rodents, or other pests. Observed two flying insects in the back dry storage room of the restaurant landing on the walls, also three flying insects near the drive doors landing on the walls , also observed 1 live fly land on single service drink carrier in dining room drink station. Spoke to operator concerning pest control service. **Admin Complaint**
08A-05-6    High Priority - Raw animal food stored over ready-to-eat food. Observed boxes raw unpasteurized shell eggs stored above tomato inside walk in cooler. Employee removed improper storage inside walk in cooler.
03A-03-4    High Priority - Shell eggs held in a unit maintaining an ambient air temperature greater than 45 degrees Fahrenheit. Checked temperature of unpasteurized shell eggs 47°f stored on the top panel of the reach in cooler per operator placed out at 9am less than 4 hours recommend quick chill the eggs inside walk in cooler. **Corrective Action Taken** **Warning**
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed rehydrate onions 71°f stored inside top panel reach in cooler on the cook line prepared on 6/24 per operator Thursday checked on next day Friday 6/25 stored inside walk in cooler over night. Issued a stop sale notice at time of inspection.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed vanilla mix (hopper) 47°f stored inside Taylor Machine located in the front service area. **Warning**
03B-01-6    High Priority - Time/temperature control for safety food hot held at less than 135 degrees Fahrenheit or above. Checked temperature of hamburger patties 131°f stored inside warmer cabinet, must maintain 135°f for hot holding. **Warning**
03D-06-5    High Priority - Time/temperature control for safety food prepared from/mixed with ingredient(s) at ambient temperature not cooled to 41 degrees Fahrenheit within 4 food due to temperature abuse. Observed onions rehydrate with warm tap water 71°f stored inside top panel reach in cooler on the cook line prepared on 6/24 per operator Thursday checked on next day Friday 6/25 stored inside walk in cooler over night. Issued a stop sale notice at time of inspection. **Warning**
41-11-4    High Priority - Toxic substance/chemical stored by or with single-service items. Observed spray bottle stored next to carry out trays and napkin dispenser in the service service area. Spray bottle removed at time of inspection. **Corrected On-Site**
22-20-5    Basic - Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine. Observed slime and mold like like growth on the seam door of the ice machine.
31A-02-4    Intermediate - Handwash sink not accessible for employee use due to being blocked by garbage can. Observed broom stored directly in front of the hand wash sink located front counter.
27-16-4    Intermediate - Hot water 74°f at employee handwash sink next to ice machine and 76°f in family restroom
