Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

14-11-4    Basic - Equipment in poor repair. Triple sink nearest to entrance to dining area leaks.
36-73-4    Basic - Floors, walls, and/or ceilings soiled/has accumulation of debris. Floor under kitchen equipment. Floor behind reach in freezer in kitchen. Floor under dry food storage shelves. Wall and door in front area by entrance to kitchen. **Repeat Violation**
33-23-4    Basic - Grease receptacle/dumpster not on proper pad/nonabsorbent surface. Blue dumpster outside back door on grass/dirt.
10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Two rice scoops by rice cookers near front counter in water 77°F. Water removed. **Corrected On-Site**
38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Light fixtures in kitchen. **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Sides and backs of cooking equipment in kitchen. **Repeat Violation**
12B-13-4    Basic - Opened employee beverage container in a cold holding unit with food to be served to customers. Multiple open drinks observed in reach in coolers in kitchen area. All drinks discarded. **Corrected On-Site**
36-02-5    Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Floor throughout kitchen. **Repeat Violation**
08A-02-6    High Priority - Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Raw fish stored on top of opened bag of French fries in kitchen reach in freezer. Items moved and stored correctly. **Corrected On-Site**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 54 rodent droppings in establishment: 41 on floor in dry food storage area in back of restaurant, eight in mop sink in mop sink closet near kitchen and five on floor in chemical closet near exit door. **Warning**
01B-03-5    High Priority - Stop Sale issued due to adulteration of food product. Three individual pork chops cooked outside on bulk smoker.

16-37-1    Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No test kit available. **Repeat Violation**
51-16-7    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Establishment uses a smoker outside of building to conduct bulk cooking and has not submitted plans for use. **Warning**
