Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

Observed 27 dead roaches in establishment: six near front door behind statue and bench, eight in cabinets at front counter where to go items are stored, two behind reach in cooler in front counter area, four behind cooking equipment in kitchen, three in cabinet under drink dispenser, one on wall behind ice machine, three in storage cabinet in dry food storage area. 14-11-4    Basic - Equipment in poor repair. Gasket for reach in cooler on cook line is torn. Two prep table shelves and legs are rusted in prep area. Front of ice machine is cracked/rusted. **Repeat Violation**
36-73-4    Basic - Floors, walls and ceilings soiled/has accumulation of debris. Floor in walk in freezer. Ceiling vents in kitchen area. **Repeat Violation**
08B-38-4    Basic - Food stored on floor. Container of oil on floor in prep area. Oil moved and stored correctly. **Corrected On-Site**
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. In walk in freezer. **Repeat Violation**

08B-12-5    Basic - Stored food not covered. In walk in freezer , ground beef patties and crabmeat.
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Marinara sauce in dry food storage area. Sauce discarded.
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license. License expired 06/01/2021. **Admin Complaint**
08A-17-6    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Raw ground beef stored over raw bacon in walk in freezer. Items moved and stored correctly. **Corrected On-Site**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed nine live roaches in establishment: five behind reach in cooler in front counter area where dressings are stored, three in dry food storage in back kitchen and one by drink dispenser in dining area. **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In walk in cooler, large pan of Alfredo sauce stored overnight 81°F. Alfredo sauce discarded. In front reach in cooler, sour cream 60°F. Sour cream was stored on expo line for approximately 30 minutes. Sour cream now stored in front reach in cooler to cool. **Corrective Action Taken**
03F-02-5    High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. Hush puppy mix near fryers has no time mark. Manager states that mix was pulled out approximately one hour prior. Mix time marked. **Corrected On-Site**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Missing on faucet by triple sink where red hose is attached. **Repeat Violation** **Admin Complaint**
01C-03-4    Intermediate - Clam/mussel/oyster tags not marked with last date served. Tags provided are missing dates indicating when last oyster was served. **Repeat Violation**
53A-05-6    Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. 02B-02-5    Intermediate - Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. Manager states steaks can be ordered undercooked, but no advisory is present. Inspector emailed advisory. **Corrective Action Taken**
46-01-4    Marked exit/path to marked exit blocked. For reporting purposes only. Exit in back of kitchen blocked by ladder. **Repeat Violation**
