Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

50-09-4    Basic - Current Hotel and Restaurant license not displayed. At time of inspection license displayed expired in 2021.
35A-03-4    Basic - Dead roaches on premises. On glue trap under storage shelf off of to the side of the prep room, 6 dead roaches. Under front register on main line two dead roaches in a trap. **Warning**
35A-20-4    Basic - Dead rodent present. In back storage area connected to prep room, underneath storage shelf with menu boxes, cleaning chemicals and clean linens, two dead rodents in a glue trap. **Warning**
14-11-4    Basic - Equipment in poor repair. Reach in cooler for pizza make table, cooler doors have exposed insulation.
36-73-4    Basic - Floor soiled/has accumulation of debris. Floors through prep area under shelves and under equipment on main cook line have a build up of dust and debris.
08B-38-4    Basic - Food stored on floor. In prep room, pizza dough stored in flour bag on floor. Operator relocated to prep table shelf. In prep room, bag of onions stored on the floor, operator relocated to storage shelf. **Corrected On-Site**
32-16-4    Basic - Only means for public to access the bathroom is through a food preparation, ware washing, or food storage area. Bathroom located in back prep room area, across from prep table and past triple sink and storage shelf with single service items.
25-05-4    Basic - Single-service articles improperly stored. Carry out pizza boxes stored on the floor. Operator relocated to proper storage. **Corrected On-Site**
14-31-5    High Priority - Nonfood-grade bags used in direct contact with food. In reach in cooler in prep room area, prepared rolls stored in grocery bag.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Total of 133 droppings observed. In back prep room area, 4 rodent droppings behind reach in freezer, 4 on top of water heater, 2 on top of storage shelf with clean towels, cleaning supplies and menu boxes, 20 droppings under cash register at front counter, 2 behind pull out metal drawers next to register, 11 droppings behind pizza make reach in cooler, 7 under pizza prep table, 29 behind pizza oven, 5 droppings behind the mop sink, 4 under the storage shelf in prep room adjacent to restroom, 43 under storage shelf along wall across from bathroom, **Warning**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Cutting board at reach in cooler on main cook line for salads has a build up of food residue staining.
51-16-7    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Behind main cook line hand wash sink has moved to opposite side of pizza oven, and triple sink has been replaced with a prep table. Triple sink adjacent to restroom still exists. Back storage room with water heater is not on plans at all. **Warning**
31B-03-4    Intermediate - No soap provided at handwash sink. At hand wash sink on main cook line by triple sink, hand soap empty. Operator provided hand soap for the sink. **Corrected On-Site**
03F-10-5    Intermediate - No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Operation using time as public health control for pizza slices and Stromboli but no written plan, operator provided new paperwork as the operation only has one of the pages for the written plan. **Corrective Action Taken**
